Medicine Management Team Lead

Job summary

To undertake a Team leader role within the medicine management team, providing assistance as required to the Practice Manager.

To ensure the medicines management team provides:

Quick, efficient and accurate provision, in the issuing of medication requests to the patients of Brampton Medical Practice, and dispatch to the relevant dispensing location.

Audits, stocktake and ordering of vaccines and medical consumables for all Brampton Medical Practice sites are undertaken within practice agreed timescales and order quantities

Effective stock rotation and correct disposal of out of date stock in liaison with the Nurse Lead

PA drugs claim processing

Vaccination programme delivery support such as, but not limited to COVID, Flu and Childhood Immunisations

Contribute to the efficiency and quality improvement of the service to both patients and clinicians by putting into place and monitoring adherence to processes that minimize the chance of error, reduce medicine wastage, and improve the quality and accuracy of the practice repeat prescriptions.

Ensure communication to and from patients, other members the primary care team and external agencies is processed accurately and within the necessary timescales.

Liaise with hospitals or pharmacies as required with regard to medications, results, clinic letters etc.

Follow up contact as required adhering to practice policies

We are part of Carlisle Rural PCN and have an excellent team of Healthcare Professionals and supportive administration teams. We are also a dispensing practice with dedicated Medicines Management Team and Dispensing Team.

Standard Check: £43.50. Deducted at £14.50 from 1st month salary and over the following 2 months

Enhanced Check: £61.50. Deducted at £20.50 from 1st month salary and over the following 2 months

Job description

Job responsibilities

Responsibility for Staff

To supervise all staff working in the Medicine Management Team, maintaining workflow to ensure a safe system of working.

To lead on recruitment training and or mentoring of Medicine Management team as required by the Practice Manager including annual appraisal

Assist in the organisation of the Medicine Management staff rotas as requested by Practice Manager, ensuring adequate staff cover at all times including the management of the teams annual leave and lieu time.

Encourage a high standard of work at all times.

Provide staff with regular feedback on their performance and development.

Report any errors involving medication issued incorrectly to the Dispensary Manager or Practice Manager.

Ensure that team members are up to date with all mandatory training and training needs are identified and managed.

Liaise with other members of the primary health care team, outside agencies and practice volunteers as required.

Manage and monitor absence within the team reporting to the Practice Manager.

Requests for repeat medication

Actively check all methods for which repeat prescriptions can be requested including requests from outside agencies.

Advocate online ordering as the preferred method for ordering.

Inform staff and patients if there are any issues by which a prescription request can be made.

All repeat requests should be processed within 24hours.

Ensure any request that cannot be dealt with by the Dispensary Admin team is forwarded to the correct destination.

Contact patients with any problem regarding their repeat prescription request or collection service following Standard Operating Procedures

Issuing Prescriptions

To manage the process for which prescriptions requests are issued within the practice.

To ensure all protocols and standard operating procedures are followed to provide the safe issue of medication.

To undertake any necessary work as may be required and appropriate to maintain a high standard and efficient repeat prescription service.

To make sure dispensing admin staff are aware of any medication supply issues and liaise with prescribers to find suitable alternatives

Co-ordinate the repeat dispensing for community pharmacies.

Regularly review and update SOPs and protocol to minimise unnecessary tasks for prescribers.

Keep all patient screen information up to date.

Alert the dispensary team or dispensing location of any dispensing directions or priorities.

Provide a complete audit trail of prescription sent to outside agencies.

Complete a monthly safety review for prescriptions issued and reason for reprints. Report any issues, key learnings and actions.

Review all critical incidents, discuss with team Practice Manager and Dispensary Manager and contribute to whole practice review. Implement changes if required

Prescription signing

To ensure prescriptions are signed routinely to supply a regular and constant supply of prescriptions to the dispensary and other outside agencies.

To alert staff of GP rota so tasks are sent to the correct person for authorisation.

Oversee the EMIS medicine management to ensure the smooth signing of Electronic prescriptions and medicine management tasks.

Systems and procedures

To have a thorough knowledge of all practice procedures and to work in accordance of written protocols including booking appointments in line with medication requests and monitoring.

To have a general understanding of all key operational responsibilities as applied to the location e.g. safety, security, maintenance of equipment etc. and report as necessary to the Operations Manager.

To take prompt action in response to any drug alert bulletins that may be received from time to time.

To maintain full and accurate records of all prescriptions issued to provide a complete audit trail.

To ensure adherence to Medicine Management standard operating procedures at all times.

Actively seek new and improved way of streamlining the issuing of prescriptions
