Job responsibilities
The practice nurse will be
responsible for the management of patients with chronic disease, including
those living in residential and nursing homes.
Provide appropriate care and treatment in conjunction
with the partners or independently as part of the practice nursing team. This
will be according to practice polices and protocols in line with the GMS contract and NMC code of conduct.
Liaise with other members of the
primary health care team and outside agencies to provide appropriate care for
the practice population
Responsible to lead partner
Participation in regular
practice meetings with partners.

- - Able to communicate effectively, both verbally and in writing, with

- - Knowledge of and ability to work to policies and procedures,
- including confidentiality, safeguarding, lone working, information
- governance and health and safety.
