Main duties of the job

- Immunisations including child imms

- Cervical Screening

- Wound Care

- Responsible for infection control

- Chronic disease management

- Provide Health education to patients and their families

- Provide patient care in accordance with established standards and protocols

Job description

Job responsibilities

The practice nurse will be responsible for the management of patients with chronic disease, including those living in residential and nursing homes.

Provide appropriate care and treatment in conjunction with the partners or independently as part of the practice nursing team. This will be according to practice polices and protocols in line with the GMS contract and NMC code of conduct.

Liaise with other members of the primary health care team and outside agencies to provide appropriate care for the practice population

Responsible to lead partner

Participation in regular practice meetings with partners.

  • - Understanding of QoF and Chronic Disease management (diabetes,
  • Hypertension, Asthma, COPD, CHD
  • - Cervical cytology
  • - Evidence of appropriate knowledge base
  • - Understanding of PGD's, PSD'S and associated policy
  • - Ability to listen, empathise with people and provide person centred
  • support in a non judgemental way.
  • - Able to communicate effectively, both verbally and in writing, with
  • people their families, carers community groups, partner agencies
  • - Knowledge of and ability to work to policies and procedures,
  • including confidentiality, safeguarding, lone working, information
  • governance and health and safety.
  • - High level of written and oral communication skills
  • - Demonstrates personal accountability, emotional resilience and
