The Advanced Clinical Pharmacist will play a vital role in delivering patient-centered care within a multi-disciplinary team. Using advanced clinical skills, including independent prescribing, they will assess, diagnose, and manage patients with various conditions, including minor illnesses and chronic diseases.
This role ensures safe and effective medication use, promotes self-care, and supports continuity of care for high-risk groups, such as those with polypharmacy and long-term conditions. The ACP will lead medicines optimisation, clinical governance, and quality improvement, collaborating across primary, community, and hospital settings.
Working closely with the practice pharmacist and technician, the ACP will enhance medication management, support audits, and improve patient outcomes. They will also assist GPs and ANPs in assessing and treating minor ailments, improving access to timely care.
This role is part of the Additional Roles Reimbursement Scheme (ARRS) within the PCN and involves in-practice work.

Work as part of a multi-disciplinary team to assess and treat patients using expert knowledge of medicines, focusing on chronic disease management and medication reviews. Take responsibility for patients with complex polypharmacy, especially the elderly, those in care homes, or with multiple co-morbidities, and support programs like STOMP.
Provide specialist advice on medicines, tackle health inequalities, and contribute to Quality and Outcomes Framework (QOF) and Enhanced Services. Develop and optimize care pathways to improve efficiency and safety, and collaborate with the nursing team to streamline chronic disease management.
Lead integration efforts with wider healthcare teams, ensure safe and effective prescribing through audits, and support medication management post-hospital discharge. Offer mentorship to pharmacy staff, engage in clinical supervision, and stay current on pharmaceutical care. Additionally, support diabetes management services where applicable

Work
as part of a multi-disciplinary team in a patient facing role to clinically
assess and treat patients using their expert knowledge of medicines for
specific disease areas.
Be
responsible for the care management of patients with chronic diseases and
undertake medication reviews to proactively manage people with complex
polypharmacy, especially the elderly, people in care homes, those with multiple
co-morbidities (frailty, COPD, and asthma) and people with learning
disabilities or autism (through STOMP Stop Over Medication Programme).
Provide
specialist expertise in the use of medicines whilst helping to address both the
public health and social care needs of patients at the PCN and to help in
tackling inequalities.
Provide
person-centred medicines optimisation and quality improvement, whilst
contributing to the Quality and Outcomes Framework (QOF), Impact and Investment
Fund (IIF) and Enhanced Services.
Develop
new pathways when necessary and work with existing ones, such as AccuRx, to
improve efficiency, safety, and quality of care.
Work
with the nursing team to create a coherent chronic disease review system by
aligning recalls and medication reviews and identifying and eliminating
duplication of work to improve efficiency and patient care.
Through
structured medication reviews, support patients to take their medications to
get the best from them, reduce waste and promote self-care.
Have
a leadership role in supporting further integration of general practice with
the wider healthcare teams (including community and hospital pharmacy) to help
improve patient outcomes, ensure better access to healthcare, and help manage
general practice workload.
Develop
relationships and work closely with other pharmacy professionals across PCNs
and the wider health and social care system.
Take
a leadership role in the clinical aspects of shared care protocols, clinical
research with medicines, liaison with specialist pharmacists (including mental
health and reduction of inappropriate antipsychotic use in people with learning
difficulties), liaison with community pharmacists and anticoagulation; and
Be
part of a professional clinical network and have access to appropriate clinical
supervision.
High
risk drug monitoring and get involved in audits, set by ICB/NHSE or towards
borough wide aims and targets. Run searches for local and national medication
alerts/shortages.
