Job summary

This is an exciting opportunity to join a thriving Primary Care Network (PCN). This is a new role generated to support our new PCN Manager in delivering the requirements of the PCN specification.

This is a role established to work with practices within the PCN, working closely with the wider practice team and clinicians to support the wellbeing and care needs of our patient population. You will have access to general advice and support and will also have clinical team oversight and supervision from the Network Manager and Practice Managers where necessary. This is a role established to support the PCN Network Manager in carrying out day-to-day activities and to provide an element of cover or source of advice in the event of their absence.

Main duties of the job

You will be part of a team of 4 GP Practices supporting the delivery of care to the highest quality and safety. As part of the team you will provide administrative support to our staff, and support patients and carers to improve their health and quality of life. You would be expected to travel to our 4 sites if necessary to manage and prioritise your own caseload, in accordance with the need and priorities of the PCN.

Farnworth and Kearsley PCN consists of 4 member Practices, all located within close proximity to each other within the Farnworth and Kearsley area of Bolton. We have a weighted patient population of approximately 39,000 patients. We have excellent working relationships across the 4 practices and are working collaboratively to address health inequalities and to provide high standards of care to our patients.

Main duties of the job;

To provide wide-ranging, proactive and efficient administrative support to Farnworth and Kearsley PCN.

Supporting the Network Manager in the day-to-day operations of the PCN and project work.

To be responsible for the administration, co-ordination and monitoring of dedicated or targeted clinical campaigns, complaints, incidents and PCN meetings.

To work closely with Network Manager, Practice Managers, Practice Administrators, Nursing and coding teams to assist the practice in meeting contractual targets.

To utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care.

To work with GPs and other primary care colleagues to identify and manage caseload of patients.

To discuss patient-related concerns with the relevant staff and be supported to follow appropriate safeguarding procedures.

To provide co-ordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care professionals.

To help people manage their needs by answering queries, making and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care.

Collate data to understand Practice capacity performance measures and look at access processes across all Practices.
