Job summary

Due to an increasing practice list size (currently approx. 5200 patients), an opportunity has arisen for an Advanced Clinical / Nurse Practitioner to join our patient centred, friendly and supportive training practice. The successful candidate should be an independent prescriber and have experience in treating minor illness. We also offer free food Friday's and no early clinic starts (clinics usually start at 9.30am)

Main duties of the job

The successful candidate should be able to diagnose and manage long-term conditions, as well as be able to treat minor illness. Ideally, you should also be able to prescribe and be able to work independently. You should be comfortable operating within your own competence, although support is always available, should it be required. You will need to be able to triage and prioritise patient needs; being able to deal with cases accordingly including treatment, referral and admission if appropriate.

About us

The practice is based in a purpose built health centre in East Lancashire, within 1 mile of the local NHS Trust. The role will complement our existing, 2 partner, 1 salaried and 2 sessional GP team. Our practice clinical team also comprises of 2 Practice Nurses and 1 Trainee Nurse Associate. We also have a Nurse Associate, Associate Psychologist, Mental Health Practitioner, 1 Pharmacy Technician and 2 Clinical Pharmacists who are employed under the PCN additional roles scheme (ARRS).There are also other community services, who operate from the health centre. We offer excellent management and administrative support and have regular clinical, MDT and practice meetings in addition to daily GP support when required. We are strongly involved in training, taking GPST2, GPST3 and FY2 placements, as well as medical students from both Manchester and UCLAN. We also take student nurse placements. We strongly value CPD and actively encourage and support ongoing training. We are a consistently high achieving QoF practice and are part of a well established Primary Care Network. We have a caring and supportive ethos, with higher than average patient satisfaction and are looking for someone who wants to help us to improve / maintain this.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Safe clinical decision making

Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role mode

Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
