Retford & Villages Primary Care Network is looking for a motivated and enthusiastic Coordinator with a focus on Digital and Social Inclusion. You will be an integral part of our multi-disciplinary team, driving positive changes in healthcare for local people.

Your role will be to help the population of Retford & Villages Primary Care Network (PCN) to access healthcare services digitally and support social connections, in addition to improving access and communication between practices and patients through digital means.

PCNs were created to help healthcare providers work together more closely and share resources. They are groups of GP practices who work in a specific area to better meet the needs of the local community. These networks are small enough to build strong relationships but large enough to offer a wide range of services.

Salary: £26,530 per year FTE (Band 4 equivalent)

Hours: Full Time 37.5 hours/week preferred. 30 hours/week considered.

Base: Retford and Villages PCN (agile working across the PCN Practices)

Term: Fixed Term 18 months

Main duties of the job

Purpose of Role

As part of the PCN team, you will help reduce health inequalities by supporting people to access primary care through digital means. You'll assist both patients and staff in using digital services, making sure everyone can take advantage of the available technology.

Your role will involve helping to run and maintain projects aimed at improving digital access, promoting these services to the community, and finding creative ways to engage with patients, practices and community partners. The PCN hopes to increase patients confidence and awareness in using digital tools to allow them control and easier access of their own healthcare management. This will also improve patients social connections and ways they can find help and advice for themselves and their families.

About our PCN:

Retford & Villages PCN comprises of 5 General Practices including Riverside Health Centre, Kingfisher Family Practice, Crown House Surgery, Tuxford Medical Centre and North Leverton Surgery. With a patient population of over 65,000 people, we are largest of the three PCNs in Bassetlaw. The area we cover is largely rural. Although the area served is relatively affluent, there are more deprived families and communities clustered around the town of Retford. We also support a higher-than -average percentage of our patients in residential or nursing care.

Main duties and objectives

Contribute to the positive promotion and increased usage of digital health tools available to people in the PCN (eg the NHS App, internet, online communication and consultation) and to encourage people in managing their own health, care and wellbeing.

Create and use social media and online platforms to communicate healthcare information to patients, improving knowledge of the PCN services available.

Build and maintain networks, working effectively with local colleagues to deliver the objectives of the role, enabling patients and services to connect.

Work with stakeholders (which includes practice staff, patients and other agencies) to identify, reduce and remove the barriers to digital inclusion for people that experience the greatest health inequalities.

Support the facilitation of workshops and task and finish groups. Use communication and presentation skills to engage people and present difficult issues clearly to a wide range of audiences.

Responsible for managing and coordinating, with support from the PCN, the delivery of digital inclusion projects across the PCN.

Responsible for monitoring and evaluating the progress of the digital inclusion project objectives and ensure that reporting of actions takes place. Then using this information to advise on, initiate and develop services as appropriate.

Support practices in improving their patient access to healthcare using digital tools, online consultation, and digital solutions to communications and service pathways

Assist practices in having up to date information on websites relating to the digital health tools access, such as NHS App, Anima, Accurx, AskmyGP.

Comply with the policies, procedures and mandatory training of the organisation to ensure safe and effective work practices.

Attending PCN, and partner, meetings where relevant.
