Job summary

Kingston Health is a busy teaching practice serving 10000 patients. We are looking to recruit an Experienced Care Navigator, Part-time . This is an exciting opportunity to join a well established and high performing practice to improve the service we offer to patients whilst supporting the GP's.

The candidates we are seeking should be experienced in delivering excellent reception support, always maintaining professionalism and discretion and be able to work independently and as part of a team. Good communication skills and attention to detail are essential along with the flexibility to cover for annual/sick leave. Knowledge of SystmOne would be an advantage.

You will have experience of working with the public, have a cheerful and polite manner, be able to multi-task and be a good team player. You must be able to always contribute to the maintenance of patient confidentiality, inside and outside the Practice.

Main duties of the job

You will need to be a good communicator with excellent interpersonal and customer service skills; especially under pressure. You will have a range of patient facing and administrative tasks, so excellent organisational skills and time management are critical. A caring, empathetic character is essential.

Your duties will include answering telephone calls, registering patients, and general admin within the Practice. It is very important to have a 'can do' attitude. It is also important to be flexible and cover shifts when required.

Kingston Health is a teaching practice with just over 10,000 patients. 4 GP Partners, 1 Physician Associate, 2 nurses, 2 HCA's ,1 Pharmacist, Care navigators and medical secretaries based over 2 sites, Wheeler Street and Holderness Road.

Job description

Job responsibilities

Greet patients and visitors to the Practice.

Book in patients and visitors in line with Practice appointments and visitors procedures.

Respond and/or redirect all patient and visitor requests accordingly and ensure unresolved and urgent matters are passed onto incoming colleagues at end of shift.

Opening incoming mail and distribute .

Booking, amending, and cancelling patient appointments.

Recording patient information onto the clinical system.

Answering incoming telephone calls, ensuring calls are documented and redirected accordingly.

Care Navigate all patients, including referring patients to outside providers.

Must be flexible to provide holiday and sickness cover on a regular basis.

Maintenance of both computerised and manual filing systems (scanning, filing etc.).

Work safely at all times in accordance with Legislative requirements and Practice Policy and Procedures.

Administration Duties

Registrations, New Patient Questionnaires and Deductions must be kept up to date.

Any other additional administration duties as required.
