Job summary
The
post holder will demonstrate courageous and critical thinking and is a caring,
compassionate and committed experienced nurse who, acting within their
professional boundaries, will provide care for the presenting patient from
initial history taking, clinical assessment, diagnosis, treatment and
evaluation of care. They will demonstrate safe, competent clinical
decision-making and expert care, including assessment and diagnostic skills,
for patients within the general practice and Primary Care Network Teams. The
post holder will commit to demonstrating critical thinking in the safe clinical
decision-making process. They will communicate and work collaboratively with
the general practice & Primary Care Network team. Meeting the needs of
patients, supporting the delivery of policy and procedures, and providing
clinical leadership to the Primary Care Network Team.
Main duties of the job

About us
Stalybridge, Dukinfield and Mossley Primary Care Network (Stalybridge PCN) is a network of GP practices lying within Tameside and Glossop. The PCN is looking for an Advanced Nurse Practitioner to work collaboratively within the Stalybridge, Mossley & Dukinfield Localities.
Stalybridge PCN is based at a central Hub and serves a population of around 43,000 patients. It is made up of 9 GP Practices and has a growing PCN Team of professionals with the aim of making healthcare more accessible in local settings. The PCN Team is comprised of a Clinical Director, a PCN Manager and PCN Operational Manager, a Complex Care Team for Frailty & Dementia, Clinical Pharmacists, First Contact Practitioners, Mental Health Nurse for Adults, Cancer Care Coordinator, Paramedic, Physician Associate and Social Prescriber, all of whom provide services across all 9 practices.

- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs, Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
