Job summary

An excellent opportunity has arisen for Ash Trees Surgery to recruit a passionate and dedicated General Practice Pharmacist to join our friendly team.T he Practice Pharmacist will work as part of the multidisciplinary primary care team to provide expert clinical medicines management and optimise patient outcomes. The post holder will support safe, effective and cost effective prescribing across the practice, with a particular focus on medication reviews, long term condition management and reducing medicines related risk.

The role includes conducting structured medication reviews, managing repeat prescribing processes, reconciling medicines following hospital discharge, and responding to medicine related queries from patients and clinicians. The Practice Pharmacist will work closely with GPs, nurses, and external pharmacy colleagues to improve prescribing quality, reduce polypharmacy, and support adherence to national guidance including NICE and MHRA alerts.

The post holder will also contribute to clinical audits, quality improvement activity, and staff education, supporting continuous improvement in medicines safety and patient care across the practice

Main duties of the job

  • Carry out clinical medication reviews to optimise treatment, reduce polypharmacy and improve patient outcomes
  • Reconcile medicines following hospital discharge and work with patients and community pharmacies to ensure continuity of care
  • Manage repeat prescribing and reauthorisation processes, including reviewing medicines that are due for review
  • Respond to medicines related queries from patients and healthcare professionals
  • Improve prescribing quality, safety and cost effectiveness through audit, PDSA cycles and formulary management
  • Implement medicines safety alerts, drug withdrawals and guidance issued by bodies such as MHRA and NICE
  • Support multidisciplinary and multimorbidity reviews as part of the wider primary care team
  • Provide medicines information, advice and training to clinical and administrative staff within the practice
  • Review relevant pathology results for patients prescribed long term or high risk medicines

Ash Trees Surgery has 14 GP Partners and 6 salaried GPs who work alongside a varied and talented multidisciplinary team. Our clinical team includes an advanced nurse practitioner, experienced practice nurses, health care support workers, paramedics, clinical pharmacists, a mental health practitioner and first contact physiotherapists. Our main site is in Carnforth, north Lancashire with 5 branch surgeries in the beautiful surrounding rural area across the Lancashire and Cumbria border

Reduce inappropriate polypharmacy and wasteful prescribing through clinical medication review

Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
