Job summary

Practice One in Bridlington is looking to recruit an experienced Practice Nurse

We are a busy, forward thinking Practice looking for an enthusiastic and forward thinking Nurse to join our highly skilled, friendly team.

The vacancy can either be part-time or full-time depending on the candidate, with flexibility if required. Hours to be normally worked over Monday to Friday with a competitive salary based on experience and qualifications.

Main duties of the job

An experienced, friendly and outgoing Practice Nurse to work alongside our existing Nursing Team.

Be able to plan and deliver high quality Integrated Nursing Care to our Patients.

To promote health and well being within the practice.

To work with Clinical and non-clinical Practice Staff in continuing to offer excellent care and high QOF achievement.

Ideally you will have experience using Systmone, although training can be given.

About us

Practice One is located in Bridlington Medical Centre, it shares the premises with two other GP surgeries.

We are a small, friendly and busy PMS Practice and our list size currently is just over 6,000 patients.

The Practice is a high QOF achiever and we have excellent, friendly, highly motivated staff with Nurse led Chronic Disease Management Clinics.

Job Description

Accountable to: The Partners of the Practice

Salary: To be confirmed

Hours of Work: Part-time worked between Monday-Friday. Hours will be based on a shift basis between 8.00am and 6:00pm (although this may be later on occasions),

Annual Leave: 5 weeks holiday plus bank holidays rising to 6 weeks plus bank holidays following 10 years completed service.

Pension Scheme: You will be entitled to join the NHS pension Scheme

Qualifications and Experience

Registered (first level) Nurse with additional post registration UKCC qualifications/status and the BSC (Hons) Community Health Nursing (Specialist Practitioner) General Practice Nursing or Nurse Practitioner Degree or ENB A33 Autonomous Practitioner Aware, Independent Nurse Prescriber.

Main Duties and Responsibilities

Clinical All of those at G Grade level and in addition:

  • To assess, diagnose and treat specific diseases using guidelines around Prodigy/NICE, but must be responsible for own professional judgement and justify rationale within own documentation.
  • Management of commonly occurring problems with appropriate referral to clinicians as and when required.
  • To initiate treatment and prescribe in line with Nurse Prescribing Guidelines.
  • Provide continuing care to Patients across the whole Practice population.
  • Undertake diagnostic health screening, health surveillance and therapeutic interventions within a broader health promotion/health strategy.
  • Medicines Management as part of the Practices Committee.
  • Management of specific diseases as identified and agreed.
  • Triage of same day appointments requests and management of minor ailments.
  • Support Practice Nurses and Health Care Assistants as needed for both clinical and managerial perspectives.
  • Occasional Practice Nurse Duties during unexpected nurse absences.

Audit & Research

  • To participate fully in the development and maintenance of clinical protocols, guidance and audits with the Practice

Care & Programme Management

  • Initiate and carry out programmes of health screening.
  • Manage programmes of care effectively through working in partnerships with others.
  • Support and enable individual, Carers, groups and communities to take action to meet their priorities.
  • Carry our Nurse Appraisals along with the named GP Partner who has responsibility for Nurse/Staff issues.

Other Duties:

  • Ensure awareness of responsibilities in relation to Health & Safety and Infection Control.
  • Maintain effective liaison with other agencies and staff concerned with patient care and with other disciplines within the Practice.
  • Any other duties as shall reasonably be required.
  • Take an active part in Clinical supervision both as a supervisor and supervisee.
  • Participate in education and training of students from all disciplines and the induction of all members of Practice Staff.
  • To regularly update knowledge and skills through personal and professional development in line with the needs of the Practice population.
  • Participate in Practice mandatory training sessions as directed.
