Upperthorpe & Ecclesall Medical Centre is looking to
recruit a full or part-time Practice Nurse
Are you a dedicated and compassionate practice nurse looking

Due to retirement, we are seeking an experienced registered
general nurse to join our friendly, supportive and forward-thinking team.
Serving two diverse communities enables staff to gain a
greater understanding of the different health needs across the city and provides
variety and development opportunities.
Main duties of the job
We are looking for a reliable, enthusiastic and motivated
Practice nurse to join our team.
The successful candidate will work as member of the practice
nurse team and will be responsible for acting within their professional
boundaries to ensure nursing services are delivered effectively to the entitled
population. Candidates will provide a wide range of nursing services in line
with current healthcare practices and standards. Applicants must have
experience in the management of Chronic disease using evidence-based
practice. Experience in childhood
immunisations and cytology would be beneficial and previous experience in
Primary care would be desirable.
The successful candidate will need to have a UK driving
license and be flexible to travel between the 2 sites.
About us
13,700 Patients: Our patient list is around 13,700 with the
numbers split almost equally between Upperthorpe and Ecclesall.
Triage System: We run a Dr first Total Triage system
Training Practice: We are a training practice and have
medical student placements/group teaching.
Systems: SystmOne and Accurx Triage
Patient Satisfaction: Our new triage is proving popular with
patients with circa 96%rating the service good or very good.
You would be joining a team of:

5 Nurses
7 salaried GPs
1 Clinical Pharmacist
1 Physician Associate
2 ANP
4 Health Care Assistants
With further support from PCN Staff in the following
roles:
Mental Health Workers
Physiotherapy
Paramedic
Social Prescribing

Job responsibilities
To work as a practice Nurse
within the organisation taking responsibility for assessing the nursing needs
of patients attending surgery with a broad range of long-term conditions.
Implement and evaluate individual
treatment plans for patients with a known long-term condition.
All duties and responsibilities
to be carried out in accordance with protocols developed by the nursing team
and in agreement with the Clinical leads.
To provide a high quality,
evidence based easily accessible service to the patients.
To provide contraceptive, sexual
health and administer Depo-Provera injections according to protocol. To ensure
patients are manged appropriately depending on their chosen method of
contraception.
To provide a cervical screening
service in accordance with national guidelines and local NHS recall system.
To provide childhood and adult
immunisations in accordance with the British vaccination schedule.
Provide travel advice and
administer vaccinations as required/ direct patients to appropriate services
To be instrumental in the support
of Infection prevention control lead and work to the IPC requirements by
providing audits, training support and evidence of compliance to the practice
managed.
Identify and manage as
appropriate, treatment plans for patients as risk of developing a long-term
condition.
Support patients to adopt health
promotion strategies that encourage patients to live healthily and apply
principles of self-care.
Provide information and advice on
prescribes/ over the counter medication in relation to long term conditions. To
review medications within scope of practice to promote adherence, to give
guidance to patients on potential side effects and dosages.
Provide wound care
To request pathology services and
obtain blood samples.
To maintain accurate records in
conjunction with legislation
You will work within your own
scope of clinical competence and agreed acceptable limits of practice.
To take responsibility for the
teaching and supervising of the health care assistant, and nursing students as
required.
To attended designated meetings
and relevant training courses keeping professionally up to date.
