You will be required to carry out the core responsibilities of the Practice Nurse role. The Nursing Team are responsible for chronic disease management, health prevention, implementing and evaluation of individual treatment plans, managing and supporting patients with long term conditions, maintaining accurate medical records and supporting the clinical team.

  • We are looking for a qualified, registered, experienced Practice Nurse who has a good understanding of qof targets

Desirable

  • Ability to record accurate clinical notes; Ability to work within own scope of practice and to understand when to refer to GPs; Knowledge of health promotion strategies; Understands the requirement for PGDs and associated policy.
