Job responsibilities

Use high-level decision-making skills.

Undertake consultation at an advanced level, using expert knowledge and clinical skills to ensure patients receive high quality care, delivered in a timely manner.

Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

Prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of practice.

Make direct referrals to secondary care and lifestyle services within locally agreed pathways.

Maintain accurate and contemporaneous records using the practice clinic system.

Request prescriptions to GPs and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

To run Diabetes Specialist Nurse Clinics within the clinics within the practice to incorporate monitoring the controls of existing Diabetes and ensuring accurate diagnosis, care and education of all new Diabetic referrals.

To provide Diabetic patients with patient centred, evidence-based care and treatment according to local protocols, guidelines and publish data from recent major Diabetes trials.

To prevent and detect early complications and other physical, social or psychological problems

To liaise with other health professionals or voluntary organisations both community and hospital based

To introduce and follow the practice of Insulin conversion, including frequent regular follow-up

To provide a safe and comprehensive service for altering Insulin dosage or type, either with or without GP reference.

To encourage patients to self-adjust their own Insulin

To participate in group consultations for diabetes

Person Specification

Qualifications

Essential

• Degree in nursing
• Diploma or equivalent in Diabetes Management

Desirable

• Prescribing Qualification
• ANP Qualification

Experience

Essential

• Recent and relevant experience of working as in a Diabetes role.
• Post registration experience (Minimum 5 years).
• Relevant experience of management of long-term conditions
• Maintenance of up-to-date skills, knowledge and understanding; and on-going competence in Diabetes.
• Evidence of capacity to work successfully, both as an autonomous clinician and as part of a team.
• Proven ability to evaluate the safety and effectiveness of others and own clinical practice.
• Interpreting and implementing local and National policies, agendas for Health.
• Experience of delivering education and training colleagues.
• Experience of supporting and education patients to manage their health needs.
