Job summary
We are looking for a dedicated and experienced diabetic nurse to provide care to our patients.
We currently work with a Lead Diabetes GP, HCAs and phlebotomists and admin support to work with patients to improve their diabetic health and health outcomes.
Main duties of the job
Provide care of newly diagnosed diabetics
Provide annual reviews to all our diabetic patients
Provide regular monitoring review of patients struggling to control their diabetes.
Provide Care summaries to patients to allow them to manage their own health decisions
Audit of diabetic outcomes when required
Provide adhoc answers to questions and support from diabetic patients (through the appointments system)
Provide expert advice to the clinicians at the practice about diabetes
Keep up to date on a changes regarding diabetic care at national and local level. Disseminate to the team new information.
About us
The Leys Health Centre has a list of 12,300 patients around 800 of which are diabetic. It has a fantastic team of 10 GPs, 4 nurses, 2 HCAs and a phlebotomist as well as a great support team one of which is dedicated to diabetes care.
We work closely with two other practices through the PCN which includes a pharmacist team who are able to provide pharmaceutical advice when needed.
