Job summary

South East Oxford Health Alliance (SEOxHA) Primary Care Network is recruiting for an experienced paramedic to be part of the Primary Care Network multidisciplinary team working 08:30 - 18:30 onMonday, Tuesday and Wednesday.

The successful candidate will be based in Temple Cowley Health Centre, and may be required to occasionally work across the South East Oxford locality, consisting of three practices to provide:

1. Comprehensive health assessment and clinical review of patients.

2. A holistic approach, personalised centred care to the patients within the PCN community.

3. Support to the wider multidisciplinary team

Main duties of the job

The post holder will provide clinical care to patients of the PCN through surgeries, clinics, triage clinics, on-call duties, home visits and relevant administrative work together with such other duties as are required of the practices.

This includes the duty to record all patient data and measurements as directed by the practices where required to maximise income under QOF, Enhanced Services and any other incentives which may be offered from time to time and to follow all protocols and policies, including those relating to referrals and prescribing.

The post holder will work with colleagues to ensure the highest standards of care for all patients, dealing with a wide range of presenting health needs in a general practice setting.

The post holder will also contribute to planning and decision making as part of the PCN and practices teams.

Responsibilities

Triage : Telephone triage of patients requesting same day appointments and other urgent queries, prioritising the needs of patients and referring on to the GPs as necessary.

To offer care complementary to that offered by the GPs and other practitioners in the Practices.

To participate in acute care consultations and make differential diagnoses using decision-making and problem-solving skills. You will be working alongside the rest of the clinical team. Patients will be allocated according to the level of clinicians experience.

To undertake visits to patients in their own homes and to nursing homes if required during urgent/ acute episodes of care with the supervision and support of GPs.

To order necessary investigations and provide treatment and care both individually and as part of the primary care team and through referral to other agencies.

To screen patients for disease risk factors and early signs of illness

To maintain accurate and comprehensive records with specific regards to confidentiality.

To monitor and evaluate standards of care through appropriate audit, taking appropriate remedial action if standards are not achieved.

To be accountable for care given within Practices policies, protocols, and procedures.

To attend appropriate clinical update training and general and specific paramedic skills and training in order to maintain knowledge and skills.

To be aware of national and local targets; initiatives; in relation to own practice and to apply these in the clinical setting.

To recognise professional boundaries and ensure these are maintained.

Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.

Contribute to the effectiveness of the team by reflecting on own and team activities and providing support for other team members.

To maintain a professional profile and maintaining professional registration where appropriate.

  • Completed the 2-year consolidation of learning period as a newly qualified paramedic.

Desirable

  • Experience of working in primary care
  • Experience using EMIS Web & Docman

Qualifications

  • Fully qualified & registered Paramedic (followed & completed the Newly Qualified Paramedic Programme (NQPP)).
  • ENP/ECP Minor Illness Level 6.
  • Completed FCP Roadmap level 1 and prepared to complete level 2 if not done so already.

Desirable

  • Completed FCP Roadmap level 2.
  • Independent prescriber.
