Job summary
We are seeking an enthusiastic experienced
Practice Nurse to work across both of our main and branch practice sites at Wootton Vale and Shortstown Surgery, respectively, as part of a Primary Healthcare Team.
The successful candidate will be:

A knowledge of a long-term condition management, (Diabetes,
respiratory conditions, hypertension etc.), would be an advantage. We are a training practice, who are keen to develop all our staff to maximise their skills and potential and, therefore, training will
be given to improve and develop skills.

qualifications. We are a training practice and, as such we operate a student nurse programme.

Main duties of the job
The Practice Nurses provide care, for the patients, from history
taking, clinical assessment, diagnosis, treatment, and evaluation of their
care. They work collaboratively with the multi-disciplinary, general
practice team to meet the needs of patients, supporting the delivery of policy
and procedures, and providing nurse care as required.
The Practice Nurse will work in accordance with the UKCC
Code of Conduct. The post holder
is expected to supervise junior staff and to educate patients and carers where
applicable.
The post holder
is expected to participate fully in quality assurance initiatives, with
particular emphasis on a 'customer first' approach. This includes treating people
as an individual first, respecting their dignity, value, self-worth and, enabling and
supporting them to live as independent a life as possible, within their local
community.
They should be proactive
in QOF and Enhanced Service management. Processing pathology results, workflow, tasks
and assisting with audits, as required, by the practice.

which we participate. We work across two sites, Wootton and Shortstown, with a
practice size of 15600 patients.
The practice demographics vary at both sites, thus
providing an interesting mix of clinical challenges. Our team comprises of
Clinical Director (Senior GP), Business director, Practice Manager, Clinical
& Nurse Lead Manager, GPs, Pharmacists, Nurses, Nurse Associate, General
Practice Assistants, and Admin staff.

Responsibilities
- Help to
co-ordinate, daily, all the activities required to deliver an effective and
efficient nursing service and keep accurate nursing records. The post holder
will also be responsible for ensuring the promotion of safety, well-being and
interests of the patients, relatives, and staff in clinical and home settings.
- Be aware of BLMK local policies, procedures and guidelines and
carry these out in all clinical areas within the surgery and for home visits
when required.
- To work alongside and deliver health
promotion skills in response to the Local Implementation Plan, National
Guidelines, BLMK and local initiatives and keep up to date with current policy.
- To contribute to the needs of the
practice regarding staffing and support. To have a flexible approach to
covering for sickness and annual leave of other nursing staff.
- To
demonstrate clinical competence and maintain a portfolio of up-to-date practice
- To
undertake training and to develop a range of skills and knowledge, as required,
ensuring the delivery of high-quality care to patients within the community,
including practice and clinical settings.

work collaboratively with the wider multi-disciplinary team, including
statutory and voluntary organizations, in the interests of patients and carers

practice computer system ensuring the Clinical Manager is advised of potential

- Be proactive in undertaking screening
of patients to support such services as Dementia, Chlamydia and Atrial
Fibrillation screening.

review of nursing protocols and contribute to practice audits.
- Use patient contacts as opportunities
to promote health education and preventative health care.
- Provide
general and specific health screenings to the practice patients (within agreed
protocols) with referral to other clinicians as necessary.

patients on general health care and minor ailments with referral to GPs as
necessary.
