About the Role:
As a Practice Nurse at ICO Health Group, you will provide high-quality nursing care, including managing chronic conditions, immunisations, wound care, cervical smears, and health promotion. You will work closely with GPs and the wider team to support the health and wellbeing of our patients.
What We are Looking For:
- Registered Nurse with NMC registration
- Experience in primary care or community nursing preferred
- Skills in chronic disease management (e.g., asthma, diabetes)
- Competent in immunisations and cervical screening
- Excellent communication and team working skills
- IT literate with the ability to use EMIS clinical system
- A proactive, patient-centered approach

Main duties of the job
Clinical Duties:
- Provide skilled nursing care and health assessments to patients.
- Manage and monitor patients with chronic diseases (e.g., asthma, COPD, diabetes, hypertension).
- Perform wound care, dressings, suture removal, and compression therapy.
- Administer vaccinations and injections (including childhood immunisations, travel vaccines, and flu jabs).
- Conduct cervical smears in line with national screening guidelines.
- Provide contraceptive advice and support (as trained/appropriate).
- Provide advice and support for lifestyle changes, including smoking cessation, weight management, and alcohol use.
- Participate in infection prevention and control measures and maintain clinical cleanliness standards.
Administrative and Team Duties:
- Maintain accurate patient records using the EMIS clinical system.
- Participate in the call/recall system for chronic disease and screening programs.
- Support Quality and Outcomes Framework (QOF) delivery and other enhanced services.
- Contribute to clinical audits and quality improvement initiatives.
- Participate in multidisciplinary team meetings and case discussions.

- IT literate and familiar with electronic health records.
Desirable Skills and Attributes:
- Experience with EMIS Web.
- Independent prescribing qualification (V300) desirable but not essential.
- Team player with excellent communication and interpersonal skills.
- Ability to work autonomously and manage time effectively.
- Commitment to professional development and ongoing learning.
Additional Information:
- Enhanced DBS check will be required.
- Participation in clinical supervision, annual appraisal, and training updates is expected.
