Job summary

We are looking for a Practice Nurse to join our diverse team. The ideal candidate will be dynamic and highly motivated with general practice experience. You will be NMC registered with strong interpersonal and communication skills and it is imperative you are a team player.

x1 vacancy for 13 hours per week working acrossWednesday 8am - 3.30pm and Thursday 8am - 5pm, days are non-negotiable due to clinical room allocation.The successful candidate will be expected to work across the two sites.

Main duties of the job

The ideal candidate will work as an integral part of the Primary Health Care Team. Working autonomously with our patients within the surgery and our practice community. This role is varied and covers all aspects of general practice nursing duties. The ideal candidates must be a team player able to adapt to changing work practices and be flexible to cover sickness and holidays. The ideal candidates must have attention to detail as accuracy is a key requirement of the role. You must also have excellent communication skills both verbal and written.

Main duties of the job

  • Womens health (cervical cytology, contraception, etc.)
  • Wound care and dressings
  • Chronic disease management (COPD & Asthma)
  • Routine and child immunisations and vaccines
  • Travel vaccinations
  • Requesting pathology tests and processing the results, advising patients accordingly
  • Knowledge and understanding of QOF
  • Knowledge and experience of working within the framework of Locally Commissioned Services and Direct Enhanced Services

The Chislehurst Partnership is a friendly high achieving suburban partnership in the London Borough of Bromley with a countryside feel. Currently operating from two sites (The Chislehurst Medical Practice & The Woodlands Practice). The Partnership is passionate about providing a friendly, supportive and caring medical service to our population. The Partnership hopes to move to a purpose-built medical centre in the future but for the time being are operating from two premises.

We have a patient list size of circa 25,000. We are a 9 Partner partnership with a team of Salaried GPs, Advanced Nurse Practitioners and a Nursing team. Our staff are friendly and hard-working and together we provide a high level of care to the local community. We are a well organised, progressive, forward-thinking Partnership. We are part of the Mottingham, Downham & Chislehurst (MDC) Primary Care Network and share across our practices a wide range of additional roles including, Clinical pharmacists, Physicians associates, Social Prescribers, a Mental Health Practitioner and Paramedic.

Job description

Job responsibilities

Main duties of the role (not an exhaustive list) :

  • Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
    • Cytology
    • Wound care
    • Removal of Sutures
    • Urinalysis
    • Travel Clinic
    • Perform ECG recordings and present results to the requester.
    • Asthma/Peak Flow Readings/Spirometry
    • To undertake chronic disease management (ie asthma, COPD) within Practice protocols, including monitoring progress, response to treatment.
    • Hypertension Management
    • Routine immunizations/Childhood immunisations and ensure vaccines are administered under patient group directions.
    • Chaperoning and assisting patients where appropriate who are being examined by another clinician
    • Assisting GPs with minor surgery and LARC clinics if necessary
    • Administer depo injections and carry out pill checks.
    • Requesting pathology tests, for example urine culture, swabs
    • Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
    • Participation in administrative systems in the Practice
    • At all times there will be a need to maintain accurate records
    • A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
    • Attend and participate in any Practice meetings when required.
    • On occasion there may be a requirement to undertake home visits
    • Anything else that is custom and practice to enable the smooth running of the Practice

Other Responsibilities

  • Recognise and work within own competence and in accordance with the national and local guidelines Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Awareness of and compliance with all relevant practice policies and guidelines
  • Reporting on Significant Events
  • Contributing to evaluation/audit and clinical standard setting within the organisation
  • Participate in regular staff training sessions to ensure development of skills and knowledge; to assess own learning needs and undertake learning as appropriate.
  • Participate in clinical, multi-disciplinary and whole team meetings.
  • Observe and comply with all appropriate statutory requirements in relation to Child and Adult Safeguarding and to be familiar with the reporting process should this be required.
  • Attending training where appropriate.
