Job summary
We are looking for a compassionate and skilled Practice Nurse to join our dedicated primary care team. This role has become available as we say goodbye to a valued Practice Nurse who has served our patients for many years and is now retiring.
In this role, you will provide a range of nursing services to support the health and wellbeing of our patient population. You will work closely with GPs, other nurses, and healthcare professionals to deliver high-quality care, including managing long-term conditions, administering immunisations, providing sexual health services, and supporting health promotion initiatives.
You will be an integral part of a multidisciplinary team, creating a welcoming and non-judgmental environment for patients, while upholding the values of care, compassion, competence, communication, courage, and commitment. Main duties of the job
The Practice Nurse provides essential nursing care to meet the needs of our patient population, including treatment room duties, managing long-term conditions, delivering immunisations and screening programmes, supporting sexual health services, and promoting overall health and wellbeing. A key part of the role is helping patients take control of their own health and working collaboratively to improve the health of the practice population.
You will be part of a multidisciplinary team of GPs, nurses, allied health professionals, and administrative staff, providing care in a welcoming, non-judgmental environment. Duties include delivering nursing treatments independently or with GPs, reviewing test results, performing home visits, supporting patient recalls, and maintaining accurate documentation.
The role also involves supporting the nursing team, assisting junior staff, contributing to training, maintaining stock, and ensuring safe use of equipment. You will follow all relevant policies and protocols, including health and safety, infection control, equality, and confidentiality, while communicating effectively with colleagues and external agencies.
