Job summary
Responsible for working with
and assisting practice teams in providing and supporting high quality clinical care
for acute and chronic problems. Implementing effective medicine management;
identifying areas for improvement and initiating and managing change.
Main duties of the job
Be able to work
flexibly and supportively in a general practice setting alongside other clinicians,
particularly existing Clinical Pharmacist colleagues.
Develop and
manage a medicines management plan and deliver patient services as determined
by practice policy and local and national guidance.
Help build the medicines
management approach in designated practices across the Network, including
delivery of training and advice and expertise to other Primary Care colleagues,
to maximise cost-effective prescribing and improve the quality of patient care.
To identify, develop
and promote quality improvements in medicines management and pharmaceutical
understanding across community pharmacy & GP practices ensuring that best
practice is adopted.
To develop and support
new ways of working and workflow redesign linked to the safe, effective, high
quality, use of medicines. by providing professional specialist medicines
management

The Role
- Act as a contact for medicines related issues within practice and contribute pharmaceutical expertise to clinical decisions.
- To contribute to discharge advice, note reconciliation, medication reviews and reauthorisations.
- To contribute to the work of delivering high quality care and effective and efficient medicine management for patients with chronic diseases.
- To undertake risk assessment/management and ensure compliance with medicines legislation and bring to the attention to all prescribers within the practice any changes to prescribing laws/guidelines.
- Liaise with prescribing colleagues in primary and secondary care to ensure consistency with borough wide prescribing strategies.
- To ensure that prescriptions are up to date, revised and changed according to patient needs or changing condition, acting promptly on requests from outside agencies.
- To liaise with secondary care regarding any medication queries relating to patients and amend patient records accordingly inc. medication specification in hospital letters.
- To carry out medication reviews of patients on repeat medicines and amend prescriptions according to patient needs or changing conditions
- To be a first source of information for patients about medicines.
- To act on any drug alerts and amend medication accordingly in consultation with the patient.
- To ensure all practice staff have access to up-to-date medicines information providing regular updating on clinical pathways, new medicines and NICE recommendations and guidelines.
- To work with the CCG medicines management team to agree target areas for the practice and amend medication accordingly in consultation with patients.
- To promote and lead medicines management campaigns to reduce medicines waste.
- To carry out prescribing audits as required and liaise with colleagues at CCG.
- Attend and participate in any Practice meetings and Network MDTs/meetings.
- To maintain accurate computerised records.
- Communicating effectively with patients and carers and recognising their needs for alternative methods of communication.
- To undertake home visits and minor ailments work when required.
- To undertake any other duties commensurate to the role, particularly supporting new ways of working inc. telephone triage and remote system working.

- Professional Registration:
- Mandatory registration with the General Pharmaceutical Council
- Qualifications:
- Pharmacy degree
- Minimum of 5 years post-qualification experience.
- The successful candidate must be willing to enrol on the CPPE Primary Care Pharmacy Education Pathway or submit evidence of exemption though past training and experience.
- Skills, knowledge and experience:
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
- An appreciation of the nature of GPs and general practices
- An appreciation of the nature of primary care
- prescribing, concepts of rational prescribing and strategies for improving prescribing
- Excellent interpersonal, influencing and negotiating skills
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Good IT skills
