PRACTICE NURSE
Responsible
to: GP Partners
Under
the supervision of:The Clinical Nursing Lead and Senior
Practice Nurse
Main
purpose of post:To provide quality nursing
services to patients within the Practice.
Hours
of work: Full or Part
time available.
Type
of Contract:Permanent subject to
a satisfactory six month probationary period
MAIN DUTIES:
The post
holder is responsible for the delivery of basic practice nursing services care
to the practice population. Supported by senior nurses within the practice,
they will deliver care within the boundaries of their role, focusing upon
supporting patients to be healthy, monitoring of long-term conditions, health
prevention and screening activities. They will work collaboratively with the
general practice team to meet the needs of patients, supporting the delivery of
policy and procedures, and providing nurse leadership as required.
PROFESSIONAL (the
duties listed below are not
exhaustive and may be subject to change/amendment as guidance changes)
To
assess and treat patients with chronic conditions and ensure the smooth running
of clinics within agreed protocols, and to ensure targets are met
Running
well person clinics and health promotion programmes by providing a holistic
assessment and lifestyle advice on diet, smoking, alcohol intake and
exercise.
Assessing
problems presented opportunistically by patients, dealing with minor illness.
Able
to document consultations according to NMC guidelines
Ensures
awareness of statutory and local clinical protection procedures, including
systems of referral. Ability to
recognise signs and symptoms of abuse.
Perform
a holistic assessment of patients attending for cervical cytology smear tests.
Perform
venepuncture according to local guidelines
Provide
a holistic approach to wound management and implement wound care in line with
current evidence based guidelines, including Doppler.
Suture
removal.
Carry
out ECG's
Offer
advice about childhood and adult vaccinations and ensure vaccines are administered
under patient group directions.
Able
to recognise and manage anaphylaxis according to current UK guidelines.
Able
to perform Cardio-pulmonary resuscitation according to current UK guidelines.
Assist
in the provision of minor surgery.
Ability
to obtain and document informed consent (either verbal or written).
Ensure
infection control guidelines are maintained.
Ability
to monitor and manage maintenance of stock and equipment to include
refrigeration and emergency equipment.
Confidentially
of information gained at work must be preserved at all times.
Chaperoning
as required.
On
occasion there may be a requirement to undertake home visits and to undertake
chronic disease management sessions on other sites within the practice boundary
DELIVERING A
QUALITY SERVICE
Contribute
to the assessment of service needs.
Assist
patients to identify their health needs.
Contribute
a nursing perspective to the practice development plan.
Manage
and organise individual patient consultations.
Aware
of identification and reporting procedures related to professional standards.
To
attend staff meetings.
Ensure
effective relationships and communications with other organisations and
agencies.
Support
the practice clinical governance agenda.
Identify
changes to clinical practice that are required to implement evidence-based
guidelines.
To
be able to facilitate change and work with the Practice Manager and GP Partners
to develop the nursing role in line with practice requirements
Ensure
that all patient contacts are recorded onto the clinical system in strict accordance with
practice protocols

professional body as appropriate e.g. NMC or HCPC, and must adhere to that
bodys rules, regulations, and codes of professional conduct. Should you not
have valid registration, this will normally be regarded as reason to terminate
your contract of employment.
