Job summary
We are expanding our Primary Care Network Well-being team and have an exciting opportunity for a patient facing Care Coordinator. You will work as part of a multidisciplinary team supporting people to navigate their care journey and ensure those most affected by health inequalities can benefit from personalised care and support.You will help patients to coordinate information and work to proactively find patients with complex conditions or needs who may benefit from extra support.
As a Patient facing Care Coordinator you will act as a point of contact between patients and the services involved in their care, supporting person centred decision making and streamlining information for both clinicians and the patient.As part of this, a care plan is developed to address the patients clinical and non-clinical needs, which could involve referring onto other members of the Multidisciplinary team.
To ensure that the changing needs of patients are met you will meet with patients, their carers and other support agencies within the practice settings or the patients home. It is expected therefore that you may be required to work alone, particularly during home visits where appropriate, ensuring you adhere to the lone working policy.
It is therefore essential that you have access to appropriate transport for this role.
Main duties of the job
Work closely within the Health and Well-being Team to assist in identifying cohorts of patients for whom personalised care plans would be beneficial and supporting patients and their families to develop tailored care and support plans.
Visiting patients within their own home or care setting to discuss their care needs.
Supporting patients to use health, social care and well-being services appropriatelyand effectively by coordinating support, signposting or referring to other services.
Sourcing and providing patients with relevant and appropriate information to
support shared decision making.
Consulting and collaborating with a range of health and social care professionals to progress plans and follow-up on any identified barriers, taking action to resolvethem.
Monitoring and evaluating patient progress and keeping accurate records.
Working closely with the practice team to carry out a range of tasks in relation to specific patient groups such as frail older people, people living with long term conditions or those needing additional support to navigate health or social care .
