Main duties of the job
Key roles and responsibilities
To deal with patient discharge letters, upgrade patient records accordingly, and update medicines.
To call the hospital if I need to confirm changes and discuss with the clinical pharmacist if needed.
To set up the patient's repeat prescription and synchronise the medicines.
Carry out drug compliance checks.
Deal with out-of-stock medicines and find suitable alternatives.
Switch inhalers for a lower carbon footprint.
To switch branded medicines to generic medicines to improve cost. I follow BNF, NICE, and GMMMG guidelines before making any changes.
To help the practice with QOF work and the primary care incentives scheme.
To perform new medicine follow-up and review with patients.
IIF (Investment and impact fund) work in practice.
Medicine monitoring and arrange blood for patients.
Send an alert message to patients to update their BP, BMI, and other information required by practice.
