Job summary

Ordsall Health Surgery is a forward thinking and friendly training GP practice with approximately 12,000 patients. We are looking to recruit a motivated and approachable Practice Nurse to join our dynamic and supportive team.

You will have excellent communication and interpersonal skills with a passion for delivering high quality, evidence-based care to our patients

Experience with chronic disease management, women's health, child and adult immunisation and health promotion is desirable.

Main duties of the job

- Delivering face to face and telephone patient care

- Assess, plan, implement and evaluate treatment programmes that promote health and well being for our practice population

- Identify and manage preventative treatment plans for patients at risk of developing a long term condition

- Work closely with patients and multidisciplinary team members to monitor and treat long term conditions

- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

-Provide care for our patients with long term conditions such as diabetes, COPD, asthma and cardiovascular diseases

- Support patients and their families to make lifestyle changes that promote the best health outcomes

- Support and manage health needs of women presenting for family planning and cervical screening

- Support the provision of vaccination programmes for adults and children, including for those with travel health requirements.

- Provide care where needed as part of the practice home visit service

About us

Ordsall Health Surgery is an inner-city practice based in Salford, Greater Manchester. We are a training practice for GPs, medical and nursing students with a list size of approximately 12,000 patients.

Our established clinical team consists of 4 GP partners, 2 salaried GPs, 3 GP registrars, 3 Pharmacists, 2 Practice Nurses, a Nurse Associate and a Health Care Assistant.

Job responsibilities

Summary of the post

The post holder is responsible for the delivery of general practice nursing to the practice population. This includes the delivery of evidence- based practice for patients with long term conditions and the provision of preventative healthcare. As an autonomous practitioner the post holder will demonstrate critical thinking and skills in clinical decision-making in the management of patients. Working in accordance with the NMC Code of Professional Conduct (2018), they will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policies and procedures, and provide nurse leadership and support as required.

Clinical Responsibilities

  • Assess, plan, and evaluate treatment programmes for patients with a known long-term condition, or those at risk of developing a long term condition.
  • Work with the full MDT to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
  • Review patient needs and treatment holistically in accordance with evidence-based practice and national and practice protocols

  • Individualise care and involve patients in decisions to support the delivery of person-centred care

  • Prioritise health problems and intervene appropriately to assist in complex, urgent or emergency situations, including initiation of effective emergency care

  • Support the care and review of patients in relation to asthma, COPD, diabetes, hypertension, cardiovascular disease, contraception, cervical cytology, adult and childhood immunisations, weight management, mental health and learning disabilities.

Communication

  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

  • Work within own competence and professional code of conduct as regulated by the NMC, ensuring maintenance of professional registration
  • Provide care that is based on up to date evidence, following local and national policy and guidance, ensuring regular updates to ensure the best care outcomes
  • Be part of practice clinical governance measures including audit, training, education and quality improvement
  • Maintain accurate record keeping for care provided
  • Complete all mandatory training packages as dictated by the practice and CCG requirements

Personal and People Development

  • To assist with the support and training of student nurses and junior nursing team members, assessing competence against set standards as requested
  • Actively promote the workplace as a learning environment for all, encouraging everyone to learn from each other and from good practice
