PCN Manager

Job summary

CWW PCN is Central and West Warrington Primary Care Network with a population of circa 54,000 patients from six GP Practices.

The post holder will work flexibly to support the PCN Clinical Director and member Practices to develop our Primary Care Network. The successful candidate will provide both operational and strategic management across the Network to achieve success in line with the PCN Network Contract Service and the priorities of the PCN.

The post holder will develop strong working relationships with a wide range of healthcare partners and stakeholders.

Main duties of the job

Taking the lead to oversee the management and the delivery of the national Primary Care Network DES requirements.

Work with and support the PCN Clinical Director to develop and deliver the PCN strategic plans and performance requirements.

Help to develop strategies to process the overall development of the PCN, specifically the maturity matrix and the PCN development plan.

Be the first point of contact for Practice Managers and Commissioners regarding PCN operations.

Develop an understanding of the Additional Roles Reimbursement Scheme (ARRS) funded roles and work with the PCN on the management, planning and deployment of these roles.

To provide line management support to the PCN employed staff, ensuring they are embedded in the PCN and co-ordinated across the PCN Practices.

Support in the budgeting and monitoring processes to support the short and long term financial stability of the network.

Chair, manage and co-ordinate PCN meetings and Terms of Reference.

Manage and monitor contractual requirements on behalf of the PCN with providers.
