Job summary
An exciting opportunity has arisen for an
enthusiastic motivated practice nurse who can offer a full range of general
practice nursing, with qualifications in chronic disease management.
The successful applicant will be instrumental in providing
pro-active support to the team and must have good clinical knowledge in their
field and understanding of General Practice.
Ideally, we are looking for 25 to 30 hours but are
open to negotiation. The position is working over 4
or 5 days and includes working until 6.30pm/8.30 pm (extended hours), for one
shift per week (Monday) and offering cover and flexibility where required. General Practice experience is essential.
Main duties of the job
We are seeking someone who can provide Immunisations, Cervical Screening and all Chronic Disease Management such as diagnosis and management of asthma and diabetes, with a good understanding of QOF.
As a training practice, there would be an expectation to maintain and develop skills in delivering education to staff and students. This position offers good learning opportunities for the right candidate.
The post will be based at Pendle View Medical Centre. The practice has 4 GP partners, with Nurse Associates, and Health Care Assistants to support the team.
We are a training Practice with Medical Students, Foundation and Speciality Trainee Doctors.
Our aim is to deliver a high standard of health care to our local practice population, and we are based in a premises in the heart of the community.

Organise and co-ordinate the provision of nursing services for the practice, to include the delegation of duties to other practice nursing staff as appropriate.
Provide nursing treatments to patients in participation with general practitioners or independently, meeting agreed protocols
Implement and evaluate individual treatment plans for patients with a known long-term condition.
Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
Provide general and specific health screenings to practice patients (within agreed protocols) with referral to general practitioners as necessary
Assist in the clinical assessment of patients, in line with practice protocols and procedures.
Implement and participate in vaccination and immunisation programmes for both adults and children.
Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc. and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs
