Job summary

The post holder will work alongside the Practices team of GP's to provide a high level of care to the 20,000 patients registered with the Practice.

The post holder will complete assessments and provide diagnosis, treatment planning and implementation.

Main duties of the job

· Conduct consultation appointments with patients, both in person and over the phone.

· Perform initial assessment and examination of patients: taking medical histories and completing physical examinations.

· Request and interpret diagnostic tests and laboratory investigations where necessary.

· Diagnose medical problems and conditions and develop treatment and management plans. Confidently explain these to the patient concerned.

· Provide direct care to patients using established clinical guidelines for both routine and urgent complaints.

· Refer patients to specialists and other health professionals where appropriate.

· Educate patients in preventative health care.

· Complete home/care home visits to patients, when required.

· Work Autonomously, under the supervision of the designated Supervisor and Practice GP's.

· Work within the remit of a Physicians Associate and know the boundaries of the role. Escalate concerns and difficult cases appropriately.

About us

MyHealth are a well-established, innovative, high achieving, training and research active practice. We have four branch surgeries located in a picturesque, semi-rural area just north of York. With a growing list of nearly 20,000 patients, we continue to develop our service and our wide and varied team.

We currently have 15 GPs who work closely with a strong team of dedicated allied health professionals, including Paramedics, Physiotherapists, Advanced Nurse Practitioners and Pharmacists. All are supported by a team of well-trained and committed administrators. We try to be as paper-light as possible and use EMIS as our clinical system.

We now have an opportunity for someone passionate about patient care to join our friendly and supportive practice. We are looking for a qualified Physicians Associate, to support our GP's and wider clinical team.

Job description

Job responsibilities

Clinical

· Dedication to own continuing professional development, regularly updating a personal development plan.

· Keeping up to date with relevant areas of medicine by completing research, reading journals, attending meetings, events, and courses.

· Re-take the UK PA National Re-certification Examination as required to maintain professional registration.

· Perform audits within the Practice, analysing data gathered and using this to drive service improvement.

· Complete independent research to drive improvements in clinical care within the Practice.

· Reflect on own practice, identifying areas of success and learning needs, working with the supervising GP to identify a plan to develop skills further.

· Work in conjunction with staff based in the Primary Care Network so that suitable patients access services offered within the PCN.

Administration

· Accurately document all aspects of patient care in the electronic patient record.

· Work in conjunction with the practices administration team to ensure administrative functions such as referrals are actioned promptly.

· Send and receive information on behalf of the Practice in relation to patients.

· Work with the Practice team to meet the objectives of the GP contract including the terms of the Quality and Outcomes Framework and CCG commissioning.

Other

· Attend the Practice induction.

· Complete all required statutory mandatory training.

· Keep up to date with changes to the Practice's polices, procedures and protocols.

· Attend regular meetings at the Practice.

· Involvement in Practice events, research, and initiatives.
