Castleton Health Centre are looking to recruit an enthusiastic and
dedicated Advanced Nurse Practitioner to join our supportive and friendly
team.
Job summary
The post
holder will be an experienced Advanced Nurse Practitioner holding the ACP
Masters or similar qualification, acting within their professional boundaries,
will provide care for the presenting patient from initial history taking,
clinical assessment, diagnosis, treatment and evaluation of care. You will
demonstrate safe, clinical decision-making and expert care for patients within
the practice and our care homes. You will be working as part of the clinical
team in the practice.
Main duties of the job
Assess, triage and provide treatment to patients or make necessary referrals to other members of the
primary care team.
Manage
undifferentiated undiagnosed conditions and identify red flags and underlying
serious pathology and take appropriate action.
Actively
take a personalised care approach to
enable shared decision-making with the presenting person.
Provide a choice of direct access to a nurse practitioner, both in Practice and
over the telephone.
Home visits.
Instigate
necessary diagnostic tests/ investigations and
interpret findings/reports.
Prescribe
safe, effective and appropriate medication whenever this is clinically appropriate.
Provide
safe, evidence-based, cost-effective, individualised patient care.
Offer
a holistic service to patients and their families, developing where appropriate
an ongoing plan of care/support, with an emphasis on prevention and self-care.
Promote
health and well-being through the use of health education,
screening and therapeutic communication skills.
Refer
patients directly to other services/agencies as appropriate.
Contribute
to the practice achieving its targets such as QOF and IIF to sustain
the high standards of patient care and service delivery.
Help
develop and set up new patient services and participate in initiatives to
improve existing patient services.

Our clinical team is made up of GP Partners,
salaried GPs, Advanced Nurse Practitioner, Practice Nurses, Nursing Associate,
Health Care Assistants and a wide variety of Primary Care Network ARRS roles.
Castleton Health Centre are based within a few miles of the M62 and have
10,200 patients on their list.
We are part of the Canalside Primary Care Network sitting within the
Heywood, Middleton and Rochdale footprint.
