Job summary

Town Gate Practice is looking to recruit a practice nurse to join our friendly team. We are looking for 30 hours a week but this is negotiable.

Previous experience in general practice is preferred with skills in chronic condition management and cytology an advantage.

Qualified registered nurses willing to undergo training will be considered if they have relevant transferable skills.

Main duties of the job

A day as a practice nurse in Town Gate is varied and may include chronic care reviews including diabetes and asthma, common ailment clinics, women's health including cytology and contraception, baby immunisation clinics, various vaccinations and wound care. Allowing for personal development in areas of interest. We give plenty of appointment time to allow excellent patient care and the recording of care given.

About us

We are a friendly and supportive practice and thrive on giving excellent patient care. We have a varied team including 7GP's, two pharmacists, one nurse practitioner, two practice nurses, three HCA's a phlebotomist and a helpful non-clinical team. We are a teaching practice for registrar doctors and fifth year medical students.

We have a patient list size of 9000 with the main surgery in Chepstow community hospital with the newly refurbished branch site less than a mile away in Sedbury. The clinical system used is Emis.

Job summary

Responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team. Delivering care within scope of competence to a varied patient population. The practice nurses are responsible for a number of clinical areas including, health promotion, chronic disease management, baby immunisations, infection prevention and control and cytology.

Primary responsibilities

The following are the core responsibilities of the practice nurse. There may be a requirement to carry out other tasks. This will be dependent upon factors such as workload, training and staffing levels:

a. To provide routine nursing care to patients as required in accordance with guidelines.

b. To support the HCA, providing guidance when necessary and support training where required.

c. To ensure the clinical safety requirements are maintained at the practice

d. To ensure all staff are aware of Infection control and lead to maintain this

e. To provide chronic disease clinics.

f. To maintain accurate clinical records.

g. To chaperone patients where necessary

h. To assist GPs with minor surgery when required

i. To prioritise health issues and intervene appropriately

j. To support the team in dealing with clinical emergencies

k. To implement vaccination programmes for adults and children

l. To deliver opportunistic health promotion where appropriate

m. To support the practice for all relevant aspects of QUAIF

n. Any other duties as requested by the practice
