Job summary

Sutton PCNs is looking to employ Dietitians under the additional roles reimbursement scheme (ARRS) to work directly with its practices to improve the experience of patients of all ages with diet-related disorders on how they can improve their health and prevent disease by adopting healthier eating and drinking habits. The Dietitian will undertake advanced health advice to patients to learn about their health goals, dietary needs, preferences and restrictions. This role will be working autonomously, but will have the support of a small team of other PCN dietitians in Sutton, as well as a multi-professional primary care team made up of doctors, nurses, clinical pharmacists, physician associate, health coaches, social prescribers and paramedics.

You will have strong experience and knowledge of working with different patient groups and needs across a diverse population. You will have excellent communication skills in addressing and providing health advice to patients, as well as educating health professionals and the public about nutrition.

Main duties of the job

The role aims to provide a weight management and cardiovascular disease prevention service to one of the PCN areas within Sutton. You will work together with Health and Wellbeing Coaches, and other PCN dietitians to provide an MDT weight management service via both group and one-to-one sessions. You will also provide cardiovascular prevention advice to patients referred via NHS health checks.

The role will also have a focus on work to address health inequalities within the borough. This will involve health promotion work and group education sessions focused on underserved communities, working alongside the wider MDT and other organisations.

The post holder will be an effective member of a multi-disciplinary team.. You will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day to day basis.

The post holder should have their own vehicle available for home visits if required with expenses remunerated in line with contractual terms.

Sutton has a population of approximately 200000 residents registered to 23 practices and there are currently 4 Primary Care Networks (PCNs); Carshalton, Cheam & South Sutton, Central Sutton and Wallington PCN; each serving a population of approximately 50000 patients. Primary care networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system. Sutton PCN is forward-looking, friendly and focused on providing a wide range of excellent healthcare services to patients in Sutton and the surrounding area.
