Job summary
The purpose of the role is to develop collaborative relationships and work in partnership with health, social care, and community and voluntary sector providers and multi-disciplinary teams to holistically support patients wider health and well-being, public health, and contributing to the reduction of health inequalities; provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input; work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN, in accordance with the needs, priorities and any urgent support required by individuals.

Health and Wellbeing Coaches will have access to ongoing supervision, skills development, and support so they are able to continue developing and gain experience.
Main duties of the job

- Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input.
- Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN, in accordance with the needs, priorities and any urgent support required by individuals
- To be the first point of contact at GP practices where coaching services will be delivered, and to receive and manage referrals. Using existing IT and MDT channels to screen patients, with an aim to identify those that would benefit most from health coaching
- To provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently.
About us
Sutton has a population of approximately 200000 residents registered to 23 practices and there are currently 4 Primary Care Networks each serving a population of approximately 50000 patients. There are also currently a total of 9 PCN Clinical Directors.
Sutton is looking to employ Health and Wellbeing Coach under the additional roles reimbursement scheme (ARRS). They will be accountable to the Primary Care Networks, working in individual PCNs and most likely allocated to specific practices within the PCN (sometimes more than one) to work closely with and support that practice team. These individuals may also work across more than one PCN in some cases where flexibility allows and for the purposes of cross cover and to ensure general practice resilience in Sutton.
