Job summary

The Role:

As a Digital Facilitator, you'll work alongside a dedicated team to assist patients in SWL GP practices to embrace digital technology. Your role will involve building relationships with practices, PCNs, and patient groups to ensure that modern general practice tools are widely adopted.

Key Working Relationships:

You'll collaborate with various stakeholders, including SWL locality Training Hubs, the Primary Care Transformation team, Change Managers, Clinical Leads, Primary Care Networks, patient groups, community organizations, and community pharmacies.

Corporate Responsibilities:

You'll support the SWL ICS Training Hub and SWL ICB Primary Care Transformation Team by effectively fulfilling the main duties of your role.

Main duties of the job

Main Duties:

Support the implementation of digital programs in PCNs and practices, helping patients embrace digital offerings in primary care.

Encourage patients to use digital tools effectively and provide feedback to practices and the SWL primary care transformation team.

Collaborate with the team to develop digital support content, such as videos and podcasts, to facilitate patient training.

Evaluate the role and impact of the program on patients.

Contribute to the development of best practices and procedures through patient feedback.

Stay updated on NHS Digital policy issues and technologies, providing training where necessary.

Deliver against objectives, prioritize workload, and meet deadlines.

Establish working relationships with local community pharmacy teams.

Overview:

General practice has come a long way in embracing information technology, and the digital GP record is now a cornerstone of patient care. In today's digital age, people expect seamless interaction with healthcare services through technology. While numerous digital strategies promote online interactions, little support is available for patients to make the most of these tools. The SWL Primary Care Transformation Programme, in partnership with SWL Training Hub, is on a mission to change that.

We're seeking enthusiastic Digital Facilitators to join our team and empower patients with the digital tools available to them. From online consultations to patient access to medical records via the NHS app, our goal is to make these technologies accessible and user-friendly for the communities we serve.

Continuous Professional Development:

You'll engage in ongoing professional development, in line with relevant professional registration or statutory requirements. Training on specific software will be provided during the induction process.
