The Practice Manager lead role is applicable to all practice managers and deputy practice managers in Carshalton.
The specified role is to work
with Sutton Primary Care Networks to support and deliver effective
collaborative opportunities for general
practice and offer best
practice services to the
patients of Sutton.
This role will provide strong communication
links between the PCN Clinical Directors, PCN Leadership Team, PCN Services
with member practices including practice
clinical, management teams, other admin and clerical staff. It
will also support NHSE, ICB & PCN policy, initiatives and projects by
delivering suitable expertise and working knowledge of primary care to enable
effective at scale working.
The post holder will support improvements in
primary care networks and across the wider health and care community to enable workforce transformation and ensure
consistent delivery of high-quality
services to patients in their PCNs and for the Borough.
Main duties of the job
- Provide effective
communications to ensure all member practices are fully up to date with the
requirements and priorities of the PCNsto enable
them to deliver effective,
consistent and best practice primary care services.
- Support the PCN Leadership Team
to spread best practice and improve quality of primary care provision across
all the practices.
- To proactively read,
understand, disseminate and contribute to work around national specifications,
local enhanced services, other projects and proposals for out of hospital and
primary care service developments
- Work with others to create effective collaborations with all areas of health and care across
Sutton and to streamline services and eliminate operational and other
issues.
- To organise and manage regular
Sutton PM forums, inviting suitable visitors, making appropriate notifications,
keeping records of attendance and following up where practices are not
regularly represented, keeping notes of meetings
and making these available at timely intervals
particularly for those unable to attend.
- Provide specific support to the PCN Leadership Team on Sutton
specific initiatives and around
specialist skills held, project topics may include things like working at
scale, workforce planning, IT, shared policies and PMS review.
- Assist and guide other
Practice Managers or their deputies, where required, to build the resilience
of the workforce and help develop skills consistently across Sutton.
About us
The Sutton Primary Care Networks is the
collaborative vehicle for the four Sutton PCNs and the former Sutton GP
Federation working together as a single entity. We are a Community Interest
Company that aims to reinvest surpluses back into general practice and primary care
for the benefit of our practices and the people who form the communities we
serve.
Our four PCNs represent all 23 GP surgeries
around Sutton. Their collective footprints cover all of the London Borough of
Sutton and its residents who are able to access and benefit from the services
and improvements offered by Primary Care Networks.
The networks will have expanded neighborhood teams which will
comprise a range of staff such as GPs, pharmacists, district nurses, community
geriatricians, dementia workers and Allied Health Professionals such as physiotherapists and podiatrists/chiropodists,
joined by social care and the voluntary sector. Under the plans, all general
practices will be aligned to a PCN, covering 30,000-50,000 patients, with local
Enhanced services funded by CCGs and provided through the new network contracts
(see below). The networks will provide the structure and funding for services
to be developed locally, in response to the needs of the patients they serve.
It is important that community pharmacy teams are fully involved in the work of
their PCN.

hostile setting and/or where expert opinion differs.

- - In-depth understanding of CQC requirements, key lines of enquiries and best practice.
