Job summary

The Sutton PCNs are looking to employ 2 Diabetic Nurses to support the Sutton Primary Care Network team in the delivery of nursing services, working as part of the PCN multidisciplinary team, delivering care within their scope of practice to patients in the ward. Sutton PCNs would like an enthusiastic and highly motivated diabetic nurse with GP Practice background experience to work at the PCN Extended Access Hub to provide diabetes clinics. The purpose of the post is to assist the GP practice team in delivering incomplete diabetic annual review and/or complete review for patients that have failed to respond to the practice. The successful candidate will deliver a high standard of patient care as a diabetic nurse in the PCN, using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base in line with the latest diabetes guidance.

The post holder will work strictly in accordance with PCN protocols, policies and guidelines. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop. You will be joining an enthusiastic and motivated team who are friendly and approachable. We are very patient focused and staff development is high on our agenda, we want to develop all staff to the highest standard.

Working Hours: Saturdays & Sundays 09.15am-1:45pm

Main duties of the job

The diabetic nurse will work within their professional boundaries;

Assist with the organisation and co-ordination of the provision of nursing services for the service.

Provide nursing treatment to patients in participation with general practitioners or independently agreed protocols for diabetes care (NICE Guidance) and relevant SWL guidance.

Be competent and confident in managing people with diabetes (primarily type 2 diabetes).

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for diabetic patients (including those on insulin) to effectively manage the patients risks.

Manage both acute and chronic conditions, integrating both drug- and non drug-based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Sutton has a population of approximately 200,000 residents registered to 23 practices and there are currently 4 Primary Care Networks (PCNs); Carshalton, Cheam & South Sutton, Central Sutton and Wallington PCN; each serving a population of approximately 50,000 patients. PCNs form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system. 1. Clinical Care Delivery

Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan.

Make professionally autonomous decisions for which he/she is accountable

Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret findings/reports

To prescribe safe, effective and appropriate medication as defined by current legislative framework

Provide safe, evidence-based, cost effective, individualised patient care

Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care

Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills

Refer patients directly to other services/agencies as appropriate

Participate in developing and delivering specialist primary care services such as long term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension).

Work with nursing, medical and health care assistant colleagues to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/Diabetes/Mental Health) are being delivered

Work with multi-disciplinary teams within the practice and across the wider CIC, to promote integrated and seamless pathways of care

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery

Participate in identification of community health needs and develop patient/family-centred strategies to address them in liaison with appropriate community health professionals and agencies

Be aware of and contribute to, planning and delivery of practice-based commissioning

2. Professional role

Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation

Participate in audits and inspections as appropriate

Work closely with the doctors and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets

Ensure that all PCN policies are fully implemented

3. Communication:

  • Communicate effectively with other team members, including the Community team,ARRS staff, PCN Manager and referral services e.g. diabetes specialist nurse team.

The post-holder will participate in any training programme implemented by the PCN as part of this employment, such training to include:

Ensure they are carrying out regular clinical audits of their own and nursing team clinics and consultations

  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
