Do you want to work for a progressive and forward thinking GP practice?

An exciting opportunity has arisen for an enthusiastic and dynamic Advanced Clinical Practitioner to join our team of 7 ACPs working across three surgery sites in Beaminster, Maiden Newton and Bridport.

We have a structured tiered approach for our ACP team with a starting rate of £26.53 per hour for Tier 1.

We would welcome newly qualified or more experienced ACPs.

Main duties of the job

Acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. The post holder will demonstrate critical thinking skills in the decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required. In order to work at this level; requirements for advanced practice must be met.

About us

Current team includes: 7 GP partners, 3 Salaried GP, 6 Advanced Clinical Practitioners, 1 Visiting Paramedic, PCN pharmacy team, PCN social prescribing team, with comprehensive PCN based frailty and community care cover.

A GP led triage hub contributes to workload management, to reduce workload and maximise clinically relevant face to face consultations. An experienced workflow team minimises document processing with pharmacist input for discharge and other medication management.

Ammonite Health Partnership is a member of Jurassic PCN, covering approximately 60% of the PCN patient population.

Clinical practice

Assess, diagnose, plan, implement and evaluate treatment/ interventions and care for patients presenting with an undifferentiated diagnosis from within your sphere of competence.

Deliver high-quality care according to evidence based practice, following agreed protocols, local and national guidelines.

Clinically examine and assess patient needs from a physiological and psychological perspective and plan care accordingly utilising critical thinking and a sound evidence base.

Assess, diagnose, plan, implement and evaluate interventions/ treatments for patients with complex needs.

Utilise your clinical supervisor/ mentor and other clinical staff appropriately to ensure safe and effective patient care, demonstrating awareness of your own limitations.

Monitor the safety and effectiveness of own clinical practice through quality assurance strategies such as the use of audit, mentor feedback, case review and peer review.

Provide safe, evidence-based, cost-effective, individualised patient care within the surgery, patients own home or other environment where patient care is carried out.

Refer patients directly to other services/ agencies as appropriate, utilising Practice and local guidelines and in a timely manner.

Proactively identify, diagnose, monitor and manage treatment plans for patients at risk of developing a long-term condition, as appropriate.

Diagnose and manage acute conditions, integrating both drug and non-drug based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and Practice protocols, and within own scope of practice. Utilise Patient Group Directions for the administration of medication as appropriate.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Provide information and advice on prescribed and over-the-counter medication on medication regimens, side-effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients and their families to adopt health promotion strategies that promote healthy living, to apply principles of self-care and to encourage uptake of services. This may include visits to local schools, community centres and other groups as necessary.

Recognise, assess and refer patients with mental health needs, as appropriate.

Support the delivery of anticipatory care plans and lead certain community services

Provide an alternative model of urgent and same day home visits for the PCN

Act sensitively, confidentially and with empathy in demonstrating understanding of wide-ranging physical and emotional needs. Take the lead in managing contentious, sensitive and delicate issues with patients, their relatives and staff, including end of life care.

Leadership

Demonstrate high levels of communication will all members of the multidisciplinary team and demonstrate leadership at all times, acting as a positive role-model for junior staff and promoting high standards of care.

Act as an agent for change and facilitate the change process whilst encouraging others to be innovative and adaptable in the approach to change.

Contribute to the effectiveness of the team by reflecting on own and team activities, taking the lead on suggesting ways to improve and enhance the teams performance.

Contribute and participate in the development of local guidelines, protocols and standards.

Ensure that the principles of infection control and local/ national policies and guidelines are applied throughout the practice, monitoring and implementing changes as required.

Participate in teaching programmes for all members of the multidisciplinary team and help to develop and maintain a culture of career-long learning at the Practice.

Act as a coach and mentor to the inter-professional team, supporting others to develop their knowledge, skills and competences in order to maximise potential.

Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.

Evidence principles of teaching and learning and promote critical reflection, feeding back relevant information to other team members in order to share good practice and inform others about current and future developments.
