population, we are looking for a new Advanced Care Practitioner to join our
team.

Who are we?
The Wilson Practice is a friendly, mutually
supportive and happy four Partner and twelve Associate GP Partnership covering
17,500 patients in a beautiful part of Hampshire. The nursing team consists of
five ANP/NP, five practice nurses and four HCAs
How we
work...
We run a total triage system using e-Consult
and telephone calls. We also have a walk- in
minor injuries unit.
We are fully engaged with our functioning
Primary Care Network, which provides Clinical Pharmacists and Physiotherapists,
Social Prescribers and MIND workers amongst others.
Main duties of the job
The primary focus of the role is to work as an autonomous practitioner providing high quality and timely, patient centred care. A key role is to undertake telephone triage and a few face-to-face appointments for those presenting with minor or acute same day problems

diagnosing and treating patients presenting with minor illness; seeing and
advising people in respect of their continuing medical and nursing needs. Total
triage is in place for all requests for treatment. There is a requirement to
see extra or emergency patients where the situation demands. The practitioner
will need to prioritise using telephone triage the needs of patients,
accordingly, making any necessary referrals for investigations in the
appropriate manner. Patients with problems needing referral to secondary care
should be discussed with the registered GP.

- Qualified Nurse Prescriber (training can be provided for the successful candidate).
