Job summary
Social prescribing empowers people to take
control of their health and wellbeing through referral to link workers who
give time, focus on what matters to me and take a holistic approach,
connecting people to community groups and statutory services for practical and
emotional support. In this role the link workers will support people with long
term conditions, and young people with complex needs to access existing groups,
ensuring that they get the right support to manage their health and wellbeing
needs.
Social prescribing can help to strengthen
community resilience and personal resilience, and reduces health inequalities
by addressing the wider determinants of health, such as debt, poor housing and
physical inactivity, by increasing peoples active involvement with their local
communities.
This position offers an excellent opportunity to join our dynamic team, located within our newly refurbished Rawling Road Surgery. The working days for this position will be Monday, Wednesday and Friday.
Main duties of the job
Take referrals from GP practices within
primary care network, including other statutory and voluntary stakeholders.
Provide personalised support to individuals,
their families and carers to take control of their long term condition and
wellbeing, live independently and improve their health outcomes. Develop
trusting relationships by giving people time. Take a holistic approach, based on the persons priorities and the wider
determinants of health. Co-produce a personalised support plan to improve
health and wellbeing, introducing or reconnecting people with long term conditions
to community groups and statutory services.
The role will require managing and
prioritising your own caseload, in accordance with the needs, priorities and
any urgent support required by individuals on the caseload.

- Minimum of 2 years experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (desirable)
- Experience in use of databases
- Experience of administrative duties
- Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
- Working in a multi-disciplinary setting where influence and negotiation is required
- Knowledge/familiarity with medical terminology
