Job responsibilities
We
are looking for dynamic and enthusiastic Physicians Associate, To provide safe
and evidence based clinical care to patients, working alongside others in the
clinical team including GPs, Practice Nurses and to include clinical
assessment, history taking, diagnosis, assessment, treatment and evaluation of
care. The post holder will demonstrate critical thinking in the decision making
process and they will work with the entire clinical team to meet the needs of
patients. The post holder will work with the supervision and support of GPs and
others as appropriate, implementing agreed management plans and following
approved protocols where necessary.
Job Description:
To undertake
clinical sessions consisting of either telephone triage appointments, face to
face appointments or both.
To conduct home
visits, both for acute and chronic presentations
To participate in
the triage and review of incoming pathology results and review of clinical
letters
To complete medical
reports to include insurance reports, occupational health and DVLA
To act
appropriately upon alerts including QOF, medicines management, local enhanced
services and reminders during a consultation
To undertake
clinical sessions during extended hours in accordance with contractual terms
and the access needs of the practice population
To support the duty
doctor by undertaking telephone triage, and provide follow up consultations for
patients as required
Respond to medical
problems presented by patients, including history taking and examination,
organising appropriate referral for investigation and treatment and ensuring
appropriate follow-up where required
Provide clear,
confident explanations to patients about their clinical management plan, and
ensuring that the plan is followed
Develop
consultation skills to enable shared patient practitioner decisions
Justify the choice
of medication and be able to understand the impact of comorbidities and other
medications, poly-pharmacy on agent choice and prognosis
Provide appropriate
preventative health care information and advice
Provide medical
support as required to patients referred by nursing staff
Adhere to the
practice prescribing formulary, local and national guidelines
Ensure the practice
EPS (electronic prescription rate, paperless prescription generation) is
maximised
Under supervision
of the GPs, make professional, autonomous decisions in relation to presenting
problems, whether self-referred or referred from other health care workers
within the practice
Record clear and
contemporaneous consultation notes on the EMIS system and to agreed standards
Collect data for
audit purposes and participate in audit and protocol review as required
Miscellaneous:
Engaging in appropriate educational and
audit activity - as defined by the Royal
College
of Physicians Faculty of Physician Associates, in order to maintain clinical
competence and performance, including 50 hours of annual CPD
Participate in an annual appraisal
Participate in teaching and training of
medical, nursing, and physician associate students and any other students as
required in future
Participate in and support other staff in
ensuring the practice meets all required
targets
Supporting the practice staff and
responding to requests for advice and assistance from the practice reception,
secretarial and nursing staff
Attend MDT, clinical and practice meetings
as required
Provide additional enhanced services that
meet the needs of the practices patient population and maximise the income
streams to the practice
Undertake additional training where
necessary to provide these enhanced services
Awareness of and compliance with all
relevant practice policies/guidelines
Meet timescales/deadlines for audits and
written returns to ensure that the Practice meets quality standards and
receives the designated funding (e.g. Quarterly Enhanced
Services
returns, annual QOF audit etc.)
