Job summary

Main duties of the job

We are purposeful in our efforts to manage GP workload and ensure sustainability, having developed a diverse and large team of support staff over recent years. A combination of the additional roles, Patient Services and Patient Administration teams provide significant support in managing the patient journey, including, but not limited to, processing incoming documents, enacting medication changes, and undertaking medication reviews. We have clinical support from a nursing team and PCN Team.

The successful candidate will provide a full range of medical services as defined in the core GMS (General Medical Services) contract and additional enhanced services. These include telephone triage leading to remote or face to face consultations, prescribing medication. Work with the practice team to achieve enhanced and locally commissioned services and QOF targets. Participate in projects within the practice.

Our Partnership is led by 3 partners, salaried GPs, nursing team, HCAs and pharmacy team along with an experienced admin team and a comprehensive management team; delivery is over a 3-site footprint.

Job description

Job responsibilities

In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including:

  • Face-to-Face consultations.
  • Prescribing, checking, and signing acute and repeat prescriptions
  • Work with the practice team to achieve enhanced service and QOF (Quality Outcome Framework) targets.
  • Completing paperwork and correspondence in a timely fashion.
  • Participate in the practice monthly clinical governance meetings.

To keep excellent clinical records including the use of clinical coding.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.

Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

Screening patients for disease risk factors and early signs of illness.

In consultation with patients and in line with current practice disease management protocols, developing care plans for health.

Providing counselling and health education.

Referring to other care providers as appropriate.

Prescribing in accordance with the Practice/ICB prescribing formulary (or generically) whenever this is clinically appropriate.

Other Responsibilities within the Organisation

Awareness of and compliance with all relevant Practice policies/guidelines, e.g., prescribing, confidentiality, data protection, health, and safety.

A commitment to life-long learning and audit to ensure evidence-based best practice.

Contributing to evaluation/audit and clinical standard setting within the organisation.

Contributing to the development of computer-based patient records.

Contributing to the summarising of patient records and Snomed-Coding patient data.

Attending training and events organised by the Practice or other agencies, where appropriate.
