Job summary

In July 2019, NHSE released the Primary Care Network (PCN) Directed Enhanced Service (DES) contract marking some of the biggest general practice contract changes in more than a decade.

Key to the ambition is the success of PCNs. Based on GP registered lists and serving geographically logical communities of between 30,000 and 50,000 people, PCNs are designed to be small enough to provide the personal care valued by patients, but large enough to have an impact on outcomes and achieve economies of scale.

As a Network Manager your focus will be on the needs of your network, working on behalf of the Clinical Director to meet the requirements of the PCN DES. We also expect you to play a key role in supporting the development of how PCNs and stakeholder partners in the Integrated Care Partnership work together. This will be both directly in relation to specific elements of the PCN DES, and more broadly as the Integrated Care Partnership in Merton develops.

Main duties of the job

Key Responsibilities and Duties:

Lead on recruitment and management of the PCN workforce

To support the PCNs with each stage of the DES focussing on new and upcoming specifications.

To monitor, manage and maximise opportunities within the Impact and Investment Fund (IIF)

Project management

To assist in creating and supporting the PCN business plans.

and any other areas as agreed by the Clinical Director.

The PCN is a forward thinking progressive network of practices with the ethos of collaboration to deliver better patient care to its population in East Merton. We employ a variety of additional roles to complement our core GP Practice teams and the successful candidate will join a friendly and goal orientated network of staff spanning the four Practices.

The PCN Managers main areas of responsibility include:

  • First point of contact for the Clinical Director and the PCNs internally and externally.
  • Management of contractual requirements.

Support the recruitment, induction and training of new and existing staff groups

Present financial plans, monitoring information and reports to the PCNs as required

Responsible, on behalf of the Clinical Director, for ensuring the PCNs maintain good financial health, operate within budget and maximise available funding and resources

Ensure financial risks are recognised and appropriate action taken

Strategy and Leadership:

Clarify objectives and priorities for network delivery in line with operational realities, financial parameters and contract requirements

Research and apply evidence from national policy, guidance and benchmarking that will influence the planning and delivery of PCN priorities

Ensure proactive stakeholder management including frontline staff, team, service, organisation, senior leaders and external bodies

Provide and build on key performance management data, as appropriate.

Development and Change:

Support and champion the development of good quality locality network plans that deliver relevant change and improvement projects that can be measured and evaluated

Oversee a programme of audits and surveys that support and evidence measures of improvement

Identify and develop appropriate tools and measures of improvement to support effective change, e.g. performance frameworks, population health data, outcome measures

Welfare:

  • Organise PCN events, including educational, locality, neighbourhood and network wide events

Relationship Management:

Manage and maintain credibility with all key contacts within the network, to foster a culture of collaboration, and promote inter-service learning to drive continuous improvement

To work as a flexible member of the PCN, providing support to other team members when necessary

Take an active role in the development and embedding of the PCNs culture, values and reputation as providers of high-quality services

Encourage supporting staff to collaborate through sharing information and intelligence across different activities

Build good relationships with all stakeholders involved in the PCN

Develop and deliver a communication strategy with reference to the networks geographical spread

Represent the network at meetings, in the community and to the media as required
