Innovations in Primary Care have a great opportunity for a Clinical Pharmacist to join Rural North Chichester (RNC) Primary Care Network (PCN).

Rural North Chichester PCN comprises of Midhurst, Pulborough, Petworth and Loxwood surgeries all based in beautiful rural settings, comprising of 38,500 Patients. We are currently looking to expand our supportive and friendly Pharmacy Team, which includes 2 Clinical Pharmacists and 2 Pharmacy Technicians. The role also comes with opportunities for continued learning and development. Specialist interests and experience with Chronic disease management are always very welcome, but not essential.

As a PCN we offer regular peer support meetings, flexible working and a percentage of work from home hours. All staff are fully supported by our PCN Manager and PCN Workforce Manager.

This is an exciting opportunity to gain a wide range of experience and make a real impact. The post holder will use their expert knowledge of medicines to provide effective patient services and contribute to the continued learning of colleagues.

Main duties of the job

The PCN Clinical Pharmacist will be responsible for a combination of patient facing work such as medication reviews, chronic disease management, prescription queries and management of medicines through transfer of care. They will also play a significant role in service and quality improvement through audits of relevant work processes, sharing of best practice and current guidance with regard to prescribing and through work in medicines optimisation.

The successful candidate will be supported in terms of clinical learning and professional development through regular access to a GP clinical supervisor, other clinical pharmacists and through a formal education pathway through the Centre of Postgraduate Pharmacy Education. As part of a growing team of Clinical Pharmacists there is also a programme of peer support events and team meetings in place.

About you

We are seeking a Clinical Pharmacist who has an Undergraduate degree in pharmacy and registration with the GPhC.

You should be working towards or possess the independent prescriber qualification and have a minimum of 2 years' experience as a pharmacist. The post holder will undertake the CPPE pathway if not already been completed.

Job responsibilities

  • Clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
  • Be prescribers, or committed to completing training to become prescribers.
  • Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, recommendation of medication changes and communication accordingly.
  • Support colleagues and patients with medication queries and monitoring.
  • Take responsibility for the care management of patients with chronic diseases.
  • Undertake clinical medication reviews to proactively manage people with complex polypharmacy.
  • Focus especially on the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.
  • Provide specialist expertise in the use of medicines, while helping to address both the public health and social care needs of patients.
  • Through the use of audit and action planning, help to tackle inequalities.
  • Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement.
  • Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services.
  • Use structured medication reviews to support patients to take their medications to get the best from them, reduce waste and promote self-care.
  • Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).
  • Work with general practice teams to ensure that practices are compliant with CQC standards where medicines are involved.
  • Take a lead role in some aspects of supporting further integration of General Practice with other healthcare teams to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
  • Specifically, contribute to the Quality Improvement Schemes (QIS) for practices (e.g. to reduce prescribing spend and unplanned admissions)
  • Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system
  • Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and anticoagulation.
  • Undertake the Centre for Pharmacy Postgraduate Education (CPPE) Primary Care Pharmacy Education Pathway.
