Job summary

This is a brand new exciting opportunity for someone with strong leadership skills and experience in delivering IT and care related Projects to join Rural North Chichester (RNC) Primary Care Network (PCN).

Rural North Chichester PCN comprises of Midhurst, Pulborough, Petworth and Loxwood surgeries all based in beautiful rural settings, comprising of 38,500 Patients.

The role has varied scope for growth and development of digital and transformational project support within our PCN. The ideal applicant will have the skills and experience to maximise the potential of this role and align it to the needs of our PCN as it evolves. In the early stages of appointment the successful candidate will be working on the following projects:

  • PCN website development linked to the Websites of the member Practices.
  • Updating of member Practices telephone systems to cloud based technology
  • Designing user guides and interactive marketing tools aimed at our Patients
  • Growing and aligning the use of our internal intranet (TeamNet) as a collaborative communication and compliance tool.

About you

The successful candidate will need to have good knowledge of Clinical Systems (preferably SystmOne), project management experience and the ability to provide training to staff within our member Practices.

Main duties of the job

Working with the PCN Manager to lead and develop a strategic plan and implement a robust digital infrastructure which enables the delivery of the Network Contract DES through contract and performance monitoring, data quality and compliance and effective communication with PCN employees, member Practices and stakeholders.

The digital and transformation lead will support with initiatives, including integrated working at neighbourhood level to improve access to patient services and enable integrated working across the PCN which drives efficiency and reduces duplication and variation across its Practices. This includes developing the digital skills and capabilities of PCN and Practice employees.

As a member of management within the network, the Digital and Transformation Lead will support colleagues in the networks Practices with change management work from time to time.

Working with the PCN board to design and iterate a digital strategic vision and identify supporting objectives.

Facilitating the board to develop a strategic plan using analytical tools and identifying external influences in order to meet the evolving needs of the PCN

Keeping up to date with the national and local PCN guidance and using this knowledge to inform digital and transformational strategy.

Contribute to the ongoing planning, monitoring, analysis and evaluation of the PCN strategic objectives.

Strategically design projects and initiatives to meet the needs of the populations health needs and health inequalities within the community of the PCN.

LEADERSHIP AND MANAGEMENT

Develop and maintain an in-depth understanding of the PCN digital infrastructure and information technology tools

Procure and manage digital tools for the PCN

Lead on the development of digital transformation bids

Ensure all projects and initiatives are fully scoped and underpinned by a quality improvement methodology.

Ensure all projects and initiatives enhance patient access

Support the delivery of IT system development within the network.

Ensuring workforce across the PCN has skills and knowledge to utilise the digital tools effectively within the PCN by supporting with the educational needs of the PCN.

GOVERNANCE

Create and maintain accurate documentation in relation to finances, standard operating procedures, contracts, and decision making.

Ensure the management of data and implementation of all projects and initiatives within the PCN are compliant with General Data Protection Regulation.

COMMUNICATION

First point of contact for Practice Managers regarding delivery of digital projects

Ensure effective communication among all PCN stakeholders

Liaise with community healthcare providers on specific programmes and projects

Actively develop and maintain the PCNs culture, values and reputation.

COMMUNITY RELATIONSHIPS

Build effective relationships with the members GP federation, facilitating appropriate access to federation support for all member Practices.

Support the growth and development of the member Practices Patient Participation Groups and Community board

Network and build effective relationships with other local PCN Managers, digital leads, ICB and other stakeholders, engaging in peer support meetings and projects.
