Job summary

Innovations in Primary Care (IPC), a federation of the GP Practices within the Coastal West Sussex area, is looking for a PCN Lead Clinical Pharmacist to work for the ChAMP Primary Care Network.

The ChAMP network is a group of 8 practices across the Chichester area. We are a large PCN with a strong voice and history of working closely together. Our larger size allows for more collaborative working and we are looking to expand our current team of Clinical Pharmacist and Pharmacy Technicians.

The PCN Lead Clinical Pharmacist will co-ordinate the work of PCN pharmacy team, ensuring maximum effectiveness in delivering on PCN priorities. They will also provide mentorship and support to pharmacists and pharmacy technicians within the PCN.

Main duties of the job

The role of PCN Lead Clinical Pharmacist is to ensure collaboration between Clinical Pharmacist and Pharmacy Technicians within the PCN team. The Lead Clinical Pharmacist will develop close links and initiatives between primary and secondary care as well as community pharmacy partners to build fruitful relationships with local practices and the wider system. The post-holder will work closely with the PCN Management team, particularly Clinical Directors and Prescribing Leads, to ensure that PCN priorities around Structured Medication Review and Medicines Optimisation are identified, communicated, and addressed. The PCN Lead Clinical Pharmacist will be a key point of contact for other pharmacy professionals within the PCN team, supporting progress in their roles through support, sharing best practice and providing a voice for the team.

Job description

Job responsibilities

  • Approximately 50 per cent of the week to be spent undertaking work to achieve all PCN DES medicine management targets including IIF;
  • Work alongside the PCN Management and wider team to identify areas of prescribing requiring improvement and provide leadership in implementing suggested changes;

  • Ensure the pharmacy team deliver the requirements of structured medication reviews and medicines optimisation;
  • Work with external partners (e.g.; CCG and community pharmacies) to support collaboration in delivery of priorities and targets. Work across organisational boundaries to engage in interagency collaboration;
  • Help the PCN Management team identify priority areas of work and provide regular updates to the PCN Board on the delivery of identified priorities for the pharmacy team;
  • Promote new ways of working and ensure shared best practice;
  • Co-ordinate workload across the team to reduce duplication, increase learning and develop team member strengths and interests;
  • Ensure effective induction of new team members and provide clinical supervision and mentorship to support team’s development and capacity
  • Build good relationships with practices and help embed the role of Clinical Pharmacist and Pharmacy Technicians to maintain clarity of direction and expectations of pharmacy team;
  • Create a supportive culture by encouraging innovation and ideas for improvement through continuous review, quality and service improvements;
  • Promote and facilitate an active and positive learning environment to enable others to achieve their potential, particularly by encouraging and supporting reflective practice;
  • Facilitate opportunities for team members to learn from each other (e.g.; through improved communication, team meetings, shadowing, etc.);
  • Support the creation and delivery of integrated and personalised care models that promotes continuity of care for patients with long-term-conditions and complex needs;
  • Lead and coordinate clinical audits through collection and use of process and prescribing data to support quality improvement projects and initiatives;
  • Lead on development of policy and procedures to improve quality of structured medication reviews and medicines optimisation;
  • Advise PCN Management team and PCN Board of prescribing and medicine management variation and put forward ways to address health inequalities and improve patient care;
  • Create, promote and support the use of audit tools to support continuous quality improvement in prescribing and medicine optimisation data across the PCN as specified in QoF/DES/QIS guidance;
  • Lead and support the promotion of health literacy projects across the PCN to help promote patients’ self-care;
  • Promote public health interventions and initiatives in the PCN to improve patients’ experience of service and health outcomes;
  • Commit to your own personal and clinical development.
