Job summary
Innovations in Primary Care (IPC), a GP Federation in Coastal West Sussex, is looking to recruit a PCN Care Coordinator for Chanctonbury Primary Care Network (PCN). The successful candidate will be based across two practices, Henfield Medical Centre and Steyning Medical Practice, and join an established multi-disciplinary PCN team consisting of pharmacists, technicians, MSK first contact practitioners, Physician Associates, Care Coordinators, Health and Wellbeing Coaches and Social Prescibers.
The successful candidate will be caring, dedicated, reliable and person-focused and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.
Main duties of the job
Care
coordinators play an important role within a PCN to proactively identify and
work with people, including the frail/elderly, early diagnosis of cancer and
end of life care patients, to provide coordination and navigation of care and
support across health and care services.
They work closely with
GPs and practice teams to manage a caseload of patients, acting as a central
point of contact to ensure appropriate support is made available to them and
their carers; supporting them to understand and manage their condition and
ensuring their changing needs are addressed.
This is achieved by
bringing together all the information about a persons identified care and
support needs and exploring options to meet these within a single personalised
care and support plan, based on what matters to the person.

- Support the clinical and social care professionals in coordinating all key activity including access to services, MDT meetings, advice and information, and ensuring health and care planning is timely, efficient and patient-centred.
- Support the setting up, coordination and manage regular multidisciplinary hub meetings, including but not limited to; Frailty and Palliative care, to ensure a smooth and coordinated approach, especially where multiple agencies are involved.
- Develop and coordinate the integrated care team hub and will hold a caseload of patients.

- NVQ level 2 or equivalent work experience in administrative or customer service (I),

- Ability to maintain effective working relationships and to promote collaborative practice with all colleagues in PGPA, PCN and the wider system
- Ability to work with minimal supervision, acting decisively and ask for help when needed

- Excellent organisation skills
- Excellent communication skills with ability to adjust to suit the audience and quickly build rapport
- Strong understanding of data protection and confidentiality
- Self-motivated and pro-active
