Job summary

A rare opportunity has arisen within our Nursing Team. We are seeking applications for a General Practice Nurse with an additional qualification and experience in managing a long-term condition in diabetes or respiratory care.

GPS Healthcare has a proven track record of supporting nurse development, from HCA apprentices through to Nurse Practitioner level. We collaborate and support BCU, UCB and SSC in offering student nurse placements and supporting apprentice programmes.

We offer team support with regular nurse meetings and cross site collaboration incorporating the wider nursing team.

We pride ourselves on a high standard of patient care and have a sustained record of high achievement in QOF targets.

Main duties of the job

Delivery of specialist clinics, evidence-based practice for patients with long-term conditions, management and preventative nursing interventions to all patients. Referrals to community based services Supporting and managing the health needs of women presenting for family planning and cervical cytology consultations Implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and where appropriate administer vaccinations for patients travelling abroad Review patient data to work with the nursing team, to achieve focused patient healthcare Active participation in achieving QOF, UO and Enhanced Services targets and recall demands To maintain mandatory training and remain current in practice

Job responsibilities

Role Purpose

To provide experienced practice nursing services to the practice population(s) and leadership at site for the nursing team.

Supported by the nursing team within the practice, the Lead Practice Nurse will deliver care within the boundaries of their role and in accordance with specific practice guidelines and protocols.

As an autonomous practitioner, the lead nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making.

They will focus upon supporting patients to be healthy by assessing their needs, providing nursing care, health education advice, screening activities, monitoring of long term conditions, implementing care in the community or other settings, maintaining clinically accurate records and running clinics.

They will work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures, and providing nurse leadership as required.
