We are growing our Care
Co-ordinators team! You will work alongside our existing Care Coordinators
within the PCN and join our multi-disciplinary team, which includes Social Prescribing
Link Workers, Clinical Pharmacists, ANPs, Paramedics, Nurses, Health Care
Assistants, Phlebotomists, GPs and a fantastic administration and management
team. Your base within the PCN will be confirmed, dependant on job role,
however you will need to travel between sites at times.
You will be
responsible for coordinating, integrating and delivering support to practice
patients. You will ensure patients have good quality information to help them
make informed choices about their care by liaising with a broad range of
medical, social and third sector services. You will ensure effective and
synchronized care is available to these patients, proactively identifying their
personalised care needs, using the available decision support aids. You
will bring together the patients identified care and support needs and explore
with them, their options to meet these into a single personalised care and
support plan (PCSP).

Main duties of the job
Build relationships with patients, their families and carers. Carry out telephone calls and reviews within
the community setting and at dedicated venues. Build and develop a robust knowledge of health, social and third sector
provision in the Solihull area
Work closely with professionals from a variety of health specialties to
coordinate personalized care for the
patients on the case load.
- Prepare proactive care plans for appropriate patients.
- Be the patients advocate to the health and social care system.
- Signpost patients to appropriate services and sources of funding and
where appropriate assist in accessing such funding.
- Form part of the call and recall team to ensure particular population
health groups receive the proactive care they require
- Work with the clinical administrative and clinical teams to provide
proactive care for patients who need screening, health promotion and/or long-term
condition monitoring and management according to a designated area. The care
coordinator team will have the ability to cross cover as required.

In partnership with other clinical teams, collaborate on improving the
quality of health care responding to local and national policies and
initiatives as appropriate.

Evaluate patients’ responses to health care provision and the
effectiveness of care.

Support and participate in shared learning across the wider organisation.

Assess the impact of policy implementation on care delivery
·
Understand and apply legal issues that support the identification of
vulnerable and abused adults and be aware of statutory vulnerable patient’s
health procedures and local guidance.
