Job summary

The role of the Care Navigator is to assist patients to ensure that they see the clinician that most meets their needs, ensuring a professional manner at all times.

Main duties of the job

Answering telephone calls and navigating patients to the correct clinician or service. Dealing with face to face enquiries.

Greeting patients and visitors

Dealing with all requests in an efficient manner

Scheduling appointments on the phone and face to face

Preparing prescriptions

Registering new patients

Dealing with tasks and docman actions

About us

Country Park Practice, South Norwood has a 3 mornings and 2 afternoons vacancy and Lennard Road Surgery has a 3 afternoons (Monday - Wednesday) vacancy for a Care Navigator. We are looking for enthusiastic, reliable, hardworking and flexible candidates to be part of our team

Our team consist of GP partners, salaried GPs, trainee GPs, practice nurses, health care assistants, pharmacists, admin, care navigators and physiotherapists
