Job summary
An exciting opportunity has arisen for an experienced and
enthusiastic Senior Practice Nurse to join our friendly team at Boultham Park Medical
Practice. The successful candidate(s) will have a special interest in chronic
disease management and would also like to further develop in leadership and
nurse management.
We are looking to enrich our team with someone who can
deliver a wide range of general practice nursing duties, infection control and work collaboratively
with GPs, practice staff, external partners, who can support population health
management and patient care. The ideal candidate(s) would be able to take on
management responsibilities for our nursing and HCA team to ensure a high level
of care is delivered to our patients.
Main duties of the job
Clinical Duties
- Provide skilled, evidence-based quality nursing care including chronic disease management
- Access, interpret and file results pertinent to nurse-led chronic disease management
- Undertake childhood and adult imms, cervical cytology, contraception / family planning reviews, wound care, travel health advice, ECGs, diabetic foot checks, leg ulcer management, and phlebotomy
- Promote health education and disease prevention
- Support patients with long-term conditions and complex care needs and refer where appropriate
Leadership & Management
- Support the nursing and HCA team, including appraisals, training, and performance management
- Further refine, implement, and review clinical policies / protocols in line with CQC and NHS standards
- Lead on nursing-related clinical governance activities.
- Ensure the effective induction and mentoring of new staff and students
- Promote a positive and supportive work environment
Other Duties
Work with the wider practice to identify opportunities for innovation /improvement
- Participate in QOF, Enhanced Services, and other contractual obligations
- To maintain the cold chain in fridge/fridges and the daily recordings.
- Support practice-level initiatives around population health and health inequalities
- To maintain accurate, comprehensive records of all consultations and treatments.
- To report any hazardous situations or defective equipment.
- Engage in professional development and maintain professional registration
About us
Boultham Park Medical Practice is an urban GP Practice located just to the south of the centre of the city of Lincoln. Our core clinical team comprises 3 GP Partners, 3 salaried GPs, 2 Advanced Nurse Practitioners, and a team of 3 Practice Nurses and 3 Healthcare Support Staff. The clinical team is supported by the reception and admin support teams.

practice that is active in research, with close ties to the University of
Lincoln. We have a strong culture of clinical supervision and clinical
governance to support our staff with regular team meetings and practice-wide

- At least 2 years post-registration experience, preferably in Primary Care
- Chronic disease management experience with ability to interpret basic test results
- Experience supervising, leadership or mentoring
