One Life
Primary Care Network (PCN) are seeking a care co-ordinator to complement our
growing team. We have an exciting
opportunity for someone who wants to work in a new role and help to develop it.
We are looking for a self-motivated individual to work across our three GP
practices as part of our Primary Care Network in Hartlepool.
Interviews will take place week commencing 23rd October 2023.

Main duties of the job
The successful
candidate will work with the GPs and other primary care professionals within
the PCN and within the community services to identify and manage a caseload of
patients with a diagnosis of cancer, and/or learning disability and also review
our patients with severe and moderate frailty.
They will provide a more joined-up and coordinated care
journey for patients, instead of each encounter with services being seen as a
single, unconnected episode of care.
The successful candidate will be organsied, self-motivated,
keen to learn and develop, hardworking with a good team ethic, along with
excellent written and verbal communication skills.

Three friendly GP Practices, working within the same
building offer this exciting opportunity for a brand new position within the
One Life Hartlepool Primary Care Network.
Our forward thinking Practices have 34,000 patients, and have a range of
Clinical Disciplines supporting the GP team, including Clinical Pharmacists,
Social Prescribing Link Workers, Mental Health Practitioner, First Contact
Practitioner and Care Co-Ordinator.
Our PCN supports a number of training
disciplines, including GP Registrars and Medical students and can offer a
strong mentoring and training approach.
