One Life Hartlepool Primary Care Network
(Bank House Surgery, Chadwick Practice and Havelock Grange Practice)
Are looking to recruit a Clinical Pharmacist to further
improve the quality of care to our patients within our Primary Care Network
The successful candidate, working with an already established Pharmacist team will work within their
clinical competencies to provide expertise
in clinical medicines management, provide face to face and telephone structured
medication review.

About us
One Life Hartlepool Primary Care Network
(Bank House Surgery, Chadwick Practice and Havelock Grange Practice)
Three friendly GP Practices, working within the same building offer
this exciting opportunity for a brand new position within this newly formed
Network. Our forward thinking Practices
have 34,000 patients, and have a range of Clinical Disciplines supporting the
GP team, including three Clinical Pharmacists and a Pharmacy Technician. Our PCN supports a number of
training disciplines, including GP Registrars, Medical and Pharmacy students
and can offer a strong mentoring and training approach.
We will
support the successful candidate in their role, including, for example becoming an independent
prescriber or gaining postgraduate qualification in clinical pharmacy.

Job responsibilities
The
post holder will work within their clinical competencies as part of a
multi-disciplinary team to provide expertise in clinical medicines management,
provide face to face and telephone structured medication review.
They
will work across the One Life Hartlepool Primary Care Network, supporting all
three Practices
The
post holder will take responsibility for areas of chronic disease management
within the practice and undertake clinical medication reviews to proactively
manage patients with complex polypharmacy, especially for older people, people
in residential care homes and those with multiple co-morbidities (in
particularly frailty, COPD and Asthma) and people with learning disabilities or
autism (Through STOMP Stop Over Medication Programme.
The
post holder will provide clinical leadership on medicines optimisation and
quality improvement and manage some aspects of the quality and outcomes
framework and enhanced services.
The
post holder will ensure that the practice integrates with community and
hospital pharmacies to help utilise skill mix, improve patient outcomes, ensure
better access to healthcare and help manage workload.

Qualifications
Essential
- Registration with General Pharmaceutical Council
- Membership of the Royal Pharmaceutical Society
Desirable
- Masters degree in Pharmacy (MPharm)
- Clinical Diploma
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society
Experience
Essential
- Excellent interpersonal, influencing and negotiating skills
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
- Demonstrates accountability for delivering professional expertise and direct service provision
- Work effectively independently and as a team member
